1.d Understanding of Need
Field and Data Review Observations
One of APP's diS<riminating tartors is our desire to understand the project issues at the proposal level, so that we can provide the most-appropriate personnel and level of effort and so that surprises are eliminated or minimized during the design process. To that end, we have identified the following observations:
o The existing northerly curb varies from 43' at the westerly end and at the new developments to 38'. The existing southerly curb varies from 43' at the residential development currently under construction to 38' everywhere else. o Based on the amount of street that has already been widened to 43', it seems very likely that a widening to the north and to the south to 43', as reflected in the Preliminary Alignment Plan included with the RFP, will be the recommended Precise Alignment alternative.
o Although there are no overhead electric poles on either side of the street to be relocated, the existing tall transmission pole at the SE corner of East Street will be a significant alignment constraint. o There are a significant number of underground electricfacilities in lincoln, especially facilities within the existing southerly parkway, that will be impacted by the widening, which will require close coordination with Anaheim Public Utilities.
o A new 12" water main was recently constructed near the street centerline. o There is a 42"-60" storm drain near the center ofthe streetthat will influence median tree placement. o There are a significant number of private overhead signs that will probably need to be relocated onto private property, unless variances can be obtained.
• There are seven locations where building overhang and building facades will extend into the proposed right-of-way, at least one of which may warrant a roadway alignment shift. o Construction is currently on-going for the new development on the south side of lincoln. o On-street parking will be lost and will be an issue to the businesses and residences.

Key Issues/Key Discriminators
Based on our review of the RFP, field reviews, and our previous experience, we have identified the following key issues:

Urban Roadway Widening Experience
APP staff is expert at providing the necessary additional lanes in significantly constrained areas while minimizing private property impacts. The Team has successfully completed the recent design of the following arterial highway widening projects in Southern California, many involving creative, money-saving right-of-way and construction cost savings:
o Whittier Boulevard in La Habra 0 Kermore Lane in Stanton o Richmond Street in EI Segundo 0 Lakewood Boulevard in Downey o La Palma Avenue/State College in Anaheim 0 Comstock Avenue in Whittier Private Property Impacts
Paying attention to the details on a property-by-property basis is critical to the success of any project, especially street widening projects in Significantly constrained urban areas. Specific areas that will need to be addressed include the following:

Building OverhanglModificatjon - After the street widening, there will be 7 buildings that will have roof or a portion of the building falade encroaching into the public right-of-way. These roof and falade encroachments will have to be eliminated or a variance granted by the City's Planning Department. As a result, we have included an arcMect with Significant experience in modifying roofs on our team as an optional task.

Parkway Width Constraint - The most-critical segment of the widening appears to be the segment just east of La Plaza. On the north Side, the 1421 building overhang is even with, and the building face is only 4' behind, the existing public right-of- 71P a g e

way. Opposite this building on the south side. the 1418 building has entry stairs to the building that are located at the existing public right-of-way. One potential solution at this location involves transitioning the roadway to the north at this location. combined with a probable narrowing of the parkway. to avoid impacting the private stairs. which are the primary entrance to this building. Curiously, the Preliminary Alignment Plan included with the RFP does reflect a horizontal alignment shift to the north, but easterly of this location, where there does not appear to be any significant constraints.

Utilities
Identifying all existing utilities within the project area and the need to carefully coordinate utilities in-place and/or relocate utilities is imperative in order to avoid costly change orders and delays during construction. Utility interferences and SUbsequent relocations are a part of any project and they must be identified early and accurately in the design process. In order to determine the potential impacts as early in the process as possible, APP has obtained atlases of the City's sewer, storm drain, water, electrical and telecommunication facilities for the length of the project. As noted previously, the primary impacts seem to be the storm drain near the center of the street which will influence potential tree planting and the electric facilities in the existing southerly parkway. It is essential to maintain coordination with all of the pertinent utilities throughout the duration of the project. Through APP's significant experience with all types of utilities and specific experience in working directly for the City's Public Utility Departments (water and electric). we have an edge on the current project.

Environmental
The City of Anaheim (City) requires that a CEQA document be prepared for the proposed widening of Lincoln Avenue. The project consists of an existing roadway surrounded by residential and commercial development and includes right-of-way acquisition and utility relocation. We expect that an Initial Study/Mitigated Negative Declaration (lS/MND) or Negative Declaration (NO) would be the appropriate document due to the anticipated ground disturbance associated with road widening and potential noise impacts to the residences along the alignment. There may be potential for biological issues associated with urban areas, such as nesting birds, which are protected by the Migratory Bird Treaty Act. In addition. some ofthe structures along the alignment are over 50 years old and therefore of historic age and will need to considered as part of the CEQA process. If historical structures are identified, the APP team will work closely with the City to avoid or minimize the impacts to a less than Significant level.

East Street Constraints
The westerly join at East Street is critical because of the lane and street widths to the west that will impact the proposed widening. but also because of the utility pole at the SE corner of the intersection. This existing large utility pole is currently south ofthe existing BCR and will be expensive to relocate. We have prepared preliminary truck turning templates for the widened lincoln curb and determined that the pole c remain in its current location with a 25' wide curb return radius. which although less than desired for an arterial intersection, is the current radius for this corner of the intersection.

Traffic
Over the past several years, residents on Evergreen Street and Larch Street have complained about excessive traffic volume and vehicular speeding associated with Lincoln Jr. High School and the cut-through traffictrying to avoid intersections on State College Boulevard and/or East Street. As such, eastbound left-turn lanes at Lincoln Avenue intersections with larch and Evergreen Streets should be sized to accommodate local demand only in orderto discourage cut-through traffic.

Client Knowledge and Client Commitment
As reflected throughout this proposal. the APP team has considerable experience working in Anaheim and is therefore familiar with City criteria, personnel and guidelines, especially as they relate to arterial widening projects. Additionally, APP has strong Client commitment and believe in adhering to the following principle: "Serve your Clients with exceptional quality and cost-effective service by providing highly qualified staff that has extensive experience in municipal engineering projects." The commitment to serve our Client's needs guides our professional endeavors and has earned APP a strong reputation in Southern California municipal engineering. 81Pa g e

of Survey, addressing any County comments and have the map recorded. Once construction has been completed, we will set final monumentation for the acquired right-of-way within the project limits.

Task 2 -Records Research and Potholing
A thorough search of the available records will be conducted to acquire relevant data to assist in the design of the prOject. The information accumulated will include as-built drawings, utility information, and horizontal and vertical control data.

The Project Team will review the project in the field and take photographs of potential problem areas or items that need special attention, focusing on the existing private property improvements that could be impacted and utility impacts. The photographs will be filed in photo albums with a written description of each picture. The albums serve as a constant reference during design and are invaluable when discussing issues at meetings. As a part of the field review, all of the existing surface topographical features per the survey data will be verified. Any discrepancies will be noted and resolved with the surveyors.

The APP team will collect record information, including:
• As-built drawings • Utility record drawings
• Right-ol-way documents • Drainage studies
• Centerline intersection ties • Prior traffic studies
We have assumed potholing of up to 12 utilities as a part of scope of services, plus an additional 30 identified in the RFP as an optional fee.

Task 3 -Traffic Study
Traffic turning movement counts as well as pedestrian and bicycle counts will be collected at all Signalized and unsignalized intersections during AM (7:00-9:00am), Noon (11:30am-1:30pm), and PM (4:00-6:00pmj peak periods, supplementing City counts. Traffic volumes will be classified as cars, trucks, pedestrian and bicyclists. On-street parking utilization on lincoln Avenue and la Plaza will be collected for three consecutive days. Concurrently, parking utilization at adjacent commercial and residential properties will be collected to determine if adequate parking spaces will be available to absorb replaced on-street parking.

Due to the planned raised median on lincoln Avenue, traffic circulation pattern will be changed significantly.

For example, current left-turns onto area driveways will be forced to make U-turns at downstream intersection, and left-turn maneuvers out of driveways will be required to turn right and make U-turns at the first median opening. As such, traffic counts will be collected at all driveways abutting lincoln Avenue and hand aSSigned to adjacent intersections and median openings. Turning movement and driveway counts will be collected concurrently.

In addition, HMM staff will undertake a comprehensive data collection effort in the field collecting traffic operations and control data such as: posted speed limits, on-street parking characteristics, utility poles, all signs, lane widths, pavement markings and legends, traffic signal and utility cabinets, pull boxes, loops and any other relevant items. Existing vehicular travel lanes will be measured to confirm field conditions as compared to the as-built plans. Existing traffic signal equipment will be surveyed to identify pole types, vehicle and pedestrian indications, and signal phasing. HMM's traffic lead will coordinate with City TMC staff and traffic signal technician to obtain signal timing plans and conduct controller cabinet inventory. Existing traffic signal pull boxes will be checked at lincoln/East intersection to verify size of conduits/fill capacity and types of existing Ole's.

In addition to the field survey, existing conditions at the intersections, as well as controller cabinets, will be photographed, labeled and presented to City staff with the first plan submittal. These photographs will aid City staff in review 01 the submittal and determination of appropriateness of our recommendations. A minimum of 12 digital photos will be taken at each intersection.

Professional Engineering Design Services for the lincoln Avenue Widening Project (from East Street to Evergreen Street) -----' - _._------=----'------------------- - Upon completion of the field survey, our traffic lead will meet with City staff to fine-tune design issues and discuss any design deficiencies as related to current design standards and practices.

Existing signalized intersection of lincoln Avenue and East Street as well as proposed signalized intersection of lincoln Avenue and La Plaza and all unsignalized intersections will be analyzed for existing, year 2018 (Le., opening year) using "Intersection Capacity Utilization" (!CU) methodology. Since the project will result in a 3" eastbound departure lane at East Street and lincoln Avenue intersection, the eastbound right-turn lane should be converted to shared thrulright· turn lane for added capacity at this congested intersection. As specified in the Request for Proposal. 1% annual traffic growth rate will be applied to existing counts to determine opening year traffic projection. Proposed intersection and mid block improvements will further be tested and analyzed using Anaheim Traffic Analysis Model (AT AM) post 2035 prOjections.

For example, the traffic study conducted in 2013 for City of Anaheim Housing Opportunities Rezoning Project Supplemental EIR, using ATAM 2012, has identified provision of a northbound right-turn lane at lincoln Avenue and East Street intersection for post 2035 traffic condition. Due to existing Southern California Edison overhead power/ines on the east side of East Street, provision of a northbound right-turn is not currently considered by the City of Anaheim.

Synchro 8 macroscopic traffic analysiS software will be used to model and analyze signalized and unsignalized intersections and mid-block openings in order to optimize traffic signal timing for existing, and year 2018 (i.e., opening year) and 2035 conditions. Left-turn pocket will be determined based on critical 95% queue lengths. Traffic analysis documenting the study intersections service levels before and after the proposed improvements will aid the City in determining optimal lane geometry and turn pockets while thriving to maximize landscaped raided medians. lincoln Avenue within the project segment is not on City's Bikeway Master Plan, and as such the proposed widening project will not provide for Class II bike lanes. Anaheim had implementation "Road Diet" concept on paralleling Broadway between East Street and State College Boulevard whereby four-lane undivided roadway was converted to a three lane segment conSisting of parking lanes, Class II bike lanes, one travel lane per direction and a center two-way left-turn lane.

A report detailing collected date and traffic analysis will be prepared and submitted for City reviews and approval.

Task 4 - Project Precise Alignment
Based on the graphic provided in the RFP, the proposed roadway improvements will result in roadway widening for the bulk of the project limits, consisting of 11-foot through lanes, 13-foot curb lanes, 16-foot medians. and 10-foot parkways, necessitating the partial taking of 16 properties.

The precise alignment will involve development and analysis of multiple alternative alignments that meet the City's objectives and incorporate the results of the traffic study, generally widening on both sides of the street, a full widening to the north and a full widening to the south. As was the case with the other Precise Alignments that APP staff has prepared, the alternatives will consider traffic study results, private property impacts, cost-effectiveness and right-of-way requirements. Since portions of the roadway have already been Widened to the ultimate width (the northwest portion and the recent residential projects) and the impacts based on widening on both sides ofthe street seem relatively minimal, it seems logical that the recommended widening will entail widening on the north and south side ofthe street.

A matrix will be prepared which provides a relative evaluation of each alternative relative to, but not necessarily limited to, the following:
• Utility Impacts • Building Impacts
• Construction Costs • Access Restrictions
• Right-of-way Costs • Parkway Widths
The alignments will show the proposed configuration of curbs, medians, right-of-way, and traffic lanes. The precise alignment plan will also show all existing driveways and right -of-way lines. Although the Preliminary Alignment Plan in the RFP reflects n lPage

an even easterly and westerly widening and is only conceptual, and other alternatives will be evaluated refinements include the following:
• As previously discussed, use truck turning templates for the NB to EB movements at East Street to determine if the existing power pole at the SE corner can remain in its current location with the widened roadway. • T ransitioning the proposed alignment a few feet to the north to avoid significant impacts to the Original Pancake House (1418 lincoln) and then transitioning back to the south after clearing this constraint. • Verify the need for the lengthy median opening across a large entire length of 11248 lincoln which is currently being redeveloped.
• (oolionallinvestigate preliminarily the ultimate alignment of the east end of this project with the current curb alignment west of State College to establish a conceptual future curb alignment, thereby establishing a preliminary alignment for the future widening to the east. During the design of the lincoln/State College intersection by APP's Project Manager, the westerly terminus of the project was established based on the future widening to the west.

Various refinements will be identified and evaluated, which will include the results of the traffic study so that appropriate left turn movements can be incorporated into the design. The evaluation of the right-of-way requirements will be accomplished by APP's civil and traffic engineers, working in conjunction with input from City staff. The analysis will inc/ude an evaluation of cost-effectiveness, functionality, and impacts to existing improvements.

Task 5 - Environmental
CEaA Initial StudvlMitigated Negative Declaration. An IS will be prepared using the approved checklist format from the City or Appendix G of the CEQA Guidelines. ECORP will prepare a description of the project including the location of the project area; a brief description of the environmental setting; an identification of environmental effects using the above-referenced checklist format; a brief substantiation of the checklist entries; and a list of references and preparers. ECORP will provide mitigation measures (if required) that can be developed using existing data. A site visit by an Environmental Scientist will be conducted. In addition, record/database searches for biological, cultural, and paleontological resources will be conducted as well as hazardous materials.

Draft IS/MND. Afterreceipt of one (1) set of integrated comments on the Administrative Oraft IS/MNO from the City, we will revise the IS/MND accordingly. The document will be mailed to addressees on the mailing list and to the State Clearinghouse.

ECORP will also prepare the Notice of Intent (NOI) to Adopt a Mitigated Negative Declaration and Notice of Completion (NOC) and file them with the Orange County Clerk and the State Clearinghouse, as appropriate.

FinaIIS/MND and MMRP. The Lead Agency (City) must consider any comments received on the MNO when making a decision on the project. A FinaIIS/MND will be prepared and will include copies of the comment letters, responses to the comments, any changes to the Draft IS/MNO, and the Mitigation Monitoring and Reporting Program (MMRP, if required). A Notice of Determination (NOD) will also be prepared and filed.

Technical Studies. In support of the IS/MND, the following three technical studies are proposed: Cultural Resources (survey and report; including a building evaluation of 22 buildings along the alignment), Air Quality/Greenhouse Gases, and Noise.

Cultural Resources. The buildings along the project alignment are a mix of single family residences and commercial properties where minor property takes for the road widening will occur. According to a preliminary review of Assessor's records and historical aerials, many ofthe buildings along the corridor are older than 50 years. These buildings need to be evaluated using California Register of Historical Resources (CRHR) eligibility criteria. A records search will be obtained from the South Central Coastal Information Center (SCCIC) at California State Fullerton. The records search will provide information about previous cultural resources investigations and previously recorded cultural resources within a.S-mile of the Project area. ECORP will requeslthat the Native American Heritage Commission (NAHC) in Sacramento conduct a search of the Sacred Lands File.

A field survey of the Project area will be conducted by qualified archaeologists. The archaeologists will survey all unpaved areas along each side of lincoln Avenue. The project area will be examined for evidence of cultural resources, including prehistoric and historic-period (i.e., over 50 years of age) archaeological deposits and features and historic-period structures or objects. If any resources are encountered, they will be recorded and mapped in detail in accordance with the standards of the California Office of Historic Preservation (OHP).

An [CORP architectural historian, meeting the Secretary of the Interior's Professional Qualifications Standards for architectural history, will complete an evaluation of up to twenty-two (22) historic-age buildings located within the lincoln Avenue Widening Project Area. The evaluation will be conducted pursuant to the requirements of CEQA. ECORP will conduct a site visit to photograph and document the buildings on appropriate oPR 523 series records pursuant to the OHP's guidelines for documenting historic properties. ECoRP will conduct archival research, using sources that may include relevant county and city records, historical aerials, historical USGS topographic maps, and Assessor's property records, to gather historical property and building information relevant to the construction and use of the buildings. ECORP will utilize information gathered from the site visit and archival research to prepare significance evaluations of the buildings. The evaluations will be included in the cultural resources technical report for the project and will contain appropriate historic and regulatory context, an outline of the methods for archival research and evaluation, detailed descriptions of the buildings, and evaluations of eligibility of each of the buildings for the CRHR.lf any of the buildings are found eligible for the CRHR, ECORP will also prepare a high-level CEQA impacts analysis to address the potential effect of the project on the historical resources.

Air Quality/Greenhouse Gases. Urban Crossroads will review aerial photos and maps to identify air quality sensitive locations in the study area that may be impacted by construction emissions. The existing conditions ofthe project study area will be evaluated; this will include gathering background air quality data, local wind patterns in the study area. and identifying applicable rules, plans, and thresholds of Significance. Construction-related air quality impacts from associated construction activities at the project site which may include demolition, import/export of fill dirt, grading, etc. will be identified.

Construction localized Significance Threshold (lST) analysis will be conducted as recommended by the SCAQMo for construction activity. The analysis will also address potential temporary construction odors.

Greenhouse gas (GHG) emissions from construction activity will be calculated. The analYSis will discuss project design/mitigation measures that will help reduce greenhouse gas emission from the proposed project. Operational impacts as a result of increased capacity on the segment will be evaluated qualitatively. The technical report will summarize the recommended air quality and greenhouse gas mitigation measures applicable to the proposed project. ~ Urban Crossroads will review applicable, Federal, State, and local Noise criteria, including the City of Anaheim Noise Element and Municipal Code, to determine appropriate noise standards and significance criteria. The Project study area will be reviewed to identify noise sensitive uses as well as the source and location of potential noise generators. This information will be used to select up to six noise monitoring locations. The noise level measurements will be collected consistent with the criteria outlined in the City's Municipal Code.

The noise study will review the off-site average daily traffic volumes and related noise levels for existing, and existing plus project based on the traffic volumes identified in Traffic Study. The future project contributions will be calculated by comparing the "with" and "without" project noise contours. A detailed noise impact and vibration analysis associated with temporary construction activities at the project site will also be provided and the impacts expected at nearby noise-sensitive uses will be estimated. The analysis will assess the construction noise levels by phase at each surrounding noise sensitive receptor location taking into account the distance and existing noise barriers. The Noise Study will recommend the appropriate mitigation and abatement measures to ensure that the noise and vibration im pacts satisfy the City of Anaheim criteria and established CEQA thresholds of significance.

Task 6 - Right-ot-Way
The scope for the right of way work is as follows and is based on acquiring sixteen partial takes and preparation of sixteen temporary construction easements (TCE):
o Record RJW Calculations: Based on the title reports provided by the City, APP will calculate the record R/W location of the impacted parcels for use in RJW engineering as described below. o Title Report Review: Based on the title reports (16) as provided by the City of Anaheim, APP will plot the easements in relation to the record R/W location to check for possible conflicts with the project widening for the partial take properties and TeE parcels only.
o Partial Take R/W Acquisition legal Descriptions: Prepare legal descriptions and exhibits for 16 "partial take" properties for right-of-way acquisition and for 16 temporary construction easements • RJW Information Notebooks: Assemble 4 individual Parcel File Books containing the following information for each of the 16 "partial take" properties for right-of-way acquisition containing: 1. R/W legal descriptions and Exhibits; 2.

Temporary construction easement descriptions and exhibits; 3. Reduced copy of the improvement plans illustrating the improvements required for the parcels in question; 4. Title reports, vesting deeds and Schedule B documents. (oDtionaIJ Should property negotiations (by City) not be settled and completed by acquisition agreements, APP will provide additional exhibits such as: right-of-way maps, aerial(s), deeds, sketches, power point presentation{s) of property(s) or CO's for legal counsel's purposes.

Task 7 - Hazardous Materials Assessment / Survey, Report and Abatement The objective of the HMA will be to evaluate whether hazardous materials or other adverse environmental conditions are present due to past or present use of the site and/or properties in the site vicinity. We have prepared the following scope of services based on information presented in the RFP. The HMA will include the following tasks:
o A site visit to visually evaluate site characteristics for possible contaminated surface soil or surface water, improperly stored hazardous materials, possible sources of polychlorinated biphenyls, and possible risks of site contamination from activities at the site. Properties within and adjoining the project site will be visually evaluated from public R/Ws only.
o A site viCinity reconnaissance to evaluate characteristics of adjacent properties for possible environmental influences on the site.
o Review of previous environmental reports conducted for the site, if provided by the client. o Obtain and review an environmental search for up to 16 properties impacted by the project. o A review of a computerized database search of readily available government and regulatory agency environmental lists for the site and for properties located within approximately 1/4 mile of the project site. The objective of the database search will be to evaluate locations where hazardous materials may have been used or stored and their possible effects on the site. On-site properties of possible concern will be further evaluated by requesting and reviewing readily available environmental documents for these properties from regulatory agencies, or by interviewing regulatory agency personnel. Our scope of work includes review of up to three agency files for site properties. Our scope of work does not include review of files for off-Site properties. locations of properties of concern will be shown on maps of the site vicinity.
o Review State of California, Department of Conservation, Division of Oil, Gas, and Geothermal Resources oil field maps and review of information provided by the California State Fire Marshal regarding oil and natural gas pipelines. • Review of site and adjacent historical land use to provide an overview of past uses that likely involved the use or storage of hazardous materials. Information that will be used to review the s~e history will include readily available historical aerial photographs, Sanborn Maps, and historic United Slates Geological Survey Topographic Maps. Ninyo & 16 1Pa ge

Moore will attempt to note historical site uses involving the use or storage of hazardous materials from the time when the site was undeveloped.
• Prepare a Phase I HMA report. The report will document work scope findings and provide a discussion of findings, conclusions, and recommendations regarding the current environmental condition of the site and recommendations for additional assessment, as appropriate. The report will be presented in an appropriate format consistent with reports prepared by Ninyo & Moore for similar projects in California.

The HMA is a screening-level historical study evaluating the current and past uses of the site and potential impacts from hazardous materials. Depending on the results of the HMA, additional subsurface evaluation may be recommended for adverse environmental conditions that are not and cannot be known at this time. Subsurface sampling or analysis is not included in this scope and will be scoped if needed.
(ootionall Asbestos Testing. Assuming building modification is necessary, a California Certified Asbestos Consultant or Site Surveillance Technician will survey and inspect the roofing and brick planter or wall areas in order to assess homogeneous areas and suspected ACMs in accordance with local, state, and federal requirements for asbestos surveys. Up to 35 suspect samples of suspected ACMs will be collected from the portions of the structures planned for removal. Analysis by the United States EnVironmental Protection Agency recommended method of Polarized Light Microscopy will be performed on the samples by an independent, sub-contracted laboratory certified by the National Voluntary laboratories Accreditation Program. Samples will be analyzed within normal turnaround times.

Painted surfaces within the roofing and brick planter or wall locations will be screened for lead content with an X-Ray Fluorescence (XRF) analyzer by a California Department of Public Health Certified lead Inspector/Assessor or a lead Sampling Technician. The XRF will be operated in accordance with the Performance Characteristic Sheet associated with the instrument.

Ninyo & Moore will prepare a limited ACM and lBP survey report to document field procedures and survey data. The report will include locations, photographs olmaterials sampled, condition and estimated quantrties of hazardous building materials, laboratory reports and chain of custody records, certification documentation of staff performing the wor~ conclusions, and recommendations.

Task 8 - Utilities
Information will be collected from all of the utility companies and added as a part of the base mapping. Coordination with the utilities will continue throughout the design of the project. A copy of the utility file will be provided to the City. Special attention will be paid to coordinating with the numerous at-grade and above-grade utilities that will need to be relocated and adjusted to grade. Based on our preliminary review of the project area, the following are some of the key utilities that will be impacted.
• The existing catch basins on both sides of the street. • Existing street lights that will have to be relocated and closely coordinated with the City's Electrical Department. • The significant amount of existing electrical facilities within the existing southerly parkway that will be impacted by the widening.

Coordination with utilities will continue throughout the design of the project. Notices will be sent to all franchise utilities that will be impacted by the project, requesting the relocation of the pertinent facilities. Three utility coordination meetings will be conducted with all affected utilities on the Project to identify and discuss schedules and necessary adjustments/relocations.

Task 9 - Pavement Evaluation and Soils Materials Report • Non-destructive pavement deflection data shall be gathered from the through travel lanes at 25-foot intervals using the Jl lS Falling Weight Deflectometer, imposing a 9,000 pound loads per California Test Method 356. • Field data collected shall be returned to the office for processing through the computer. This processing shall produce a typed tabulation of all deflection measurements and field notes thereon, which locate the intersections of cross 17 iPage

streets, pertinent landmarks, field conditions, etc. The tabulation shall include the reading of all sensors, as well as an equivalent Traveling Deflectometer at the loading point. • An evaluation of the present pavement thicknesses utilizing component analysis with R-value strength data will be combined with the traffic index provided by the City for design of alternative replacement sections. • LaBelle Marvin will perform the appropriate laboratory testing determination of classification, natural moisture content of soils, field densities of undisturbed samples, laboratory maximum density, and HR" value. Additionally, agronomy/soil fertility testing will be provided.
• Structural sections recommendations will be provided for the new pavement areas and alternative rehabilitation methods for the existing street areas.
• A geotechnical report will be prepared addressing results of the site inspections, field sampling, laboratory testing, R Value testing, structural section/overlay recommendations, and prioritized listing of pavement rehabilitation methods with cost benefit determinations considered.

Task 10 - Roadway
In addition to the other plans described below, street improvement plans (20-scale) will be prepared using the aerial topographic survey data to create base sheets. The street improvements will be based on providing the widths and alignment as recommended in the approved Project Precise Alignment. We are anticipating the potential for separate plan and profile sheets due to the width of the roadways involved.

The street improvements will consist of two parts: 1) modifying the existing pavement as necessary to make the horizontal and vertical alignment work and 2) the widening of the existing roadway. The use of cross-sections will be critical to assure that reasonable cross-falls are maintained for the new roadway surface. Based on the proposed pavement rehabilitation strategy for each reach of roadway in conjunction with the cross-section data, the proposed roadway cross-fall will be evaluated to ensure that they are within an acceptable range.

For the portion of the roadway to be widened, one of the key issues will be to minimize the impacts to existing development by attempting to join the existing grades at the proposed right-of-way line as closely as possible. As is the case for the rehabilitation areas, design cross-sections will be critical to the design of the Widened curb and new roadway cross-falls as well as to ensure that driveway slopes are reasonable.

Task 11 - Traffic Signal
With an added third lane per direction of travel on lincoln Avenue, traffic will be dispersing onto three lanes, resulting in left turning vehicles tending to accept shorter gaps in opposing traffic at lincoln Avenue. Research indicates that accidents involving left-turn vehicle and oppOSing thru move, on average, accounts for 18% of accidents at intersections with permissive left-turns. During the early stage of design activities, HMM will conduct an eastbound/westbound left-turn phase warrant analysis for the proposed signalized intersection of lincoln Avenue and La Plaza for opening year traffic condition.

During construction of the new signal at Lincoln Avenue and La Plaza intersection, the existing mid-block signalized pedestrian crossing will be maintained or temporary signal will be installed, if required, in order to maintain Signalized school crossing at all times.

Preliminary (i.e., 50%) plans will be designed utilizing City's design standards/special provisions, latest editions of the State of California Standard Plans and SpeCifications and California MUTCD. The 50% plans for traffic signal modification design will depict but not limited be to the following: existing and proposed striping/markings, traffic signal conduits, pull boxes, pole schedule, conductor schedule, controller, potential utility conflicts, existing and proposed phasing diagrams, and notes.

There is an existing fiber optic communication line on Broadway between Manchester and State College Boulevard. Based on consultation with City staff, HMM will design a new 2.5" conduits with 24-strand fiber line on lincoln Avenue between East l8 1Pa g e
Professional Engineering Design Services for the Lincoln Avenue Widening Project (from East Street to Evergreen Street)
Street and State College Boulevard in order to provide a redundant communication line. Network redundancy reduces downtime due to cable or equipment failures in the field. Traffic interconnect plans will be prepared at 1 "=40' scale.

Task 12 - Street lighting
Currently Anaheim Public Utilities Department is using cobra head luminaries on octagon marble light poles within the project limits. Once intersection safety lighting at signalized intersections are designed and approved by the Traffic Engineering Section as part of traffic signal modification effort, the spacing of lighting fixtures on the side(s) impacted by roadway widening will be designed per City standards using AGI32 photometric software to evaluate the average foot candle (fc), minimum fc, and lighting uniformity on lincoln Avenue. Once the design is reviewed and approved, the City's Public Utilities Department will provide for conductor and pull box design of new installations.

Task 13 - Signing and Striping
Signing and striping plans will be prepared in accordance with the latest edition of the State of California Manual of Uniform Traffic Control Devices (MUTCD), City's Standard Plans and Special Provisions. Left-turns at intersections and mid-block openings will be sized in accordance with traffic analysis findings. Due to the addition of a 3" eastbound departure lane at East Street and Lincoln Avenue intersection, striping plans will show removal of lane drop arrows east of the railroad overpass and convert the eastbound right-turn lane to a shared thrulright-turn lane. A revised crosswalk at East/Lincoln and new crosswalks at La Plaza/lincoln intersections will be designed per City's Standard OetaiI477, as school crOSSings. Since striping transitions and tapers will be designed based on the 85th percentile speed, or if recent speed survey is not available, 10 MPH over the posted speed limit. Signing and striping plans will be prepared at 1 "=40' scale.

Task 14 - Traffic (ontrol
Traffic control plans will be designed considering all aspects of the work area, including sight distance, traffic speed, volume, road approaches, work duration, and the type of work. Traffic control plans (TCPs) will attempt to minimize traffic impacts, using construction staging that would allow for at least one lane per direction of travel on lincoln Avenue and all side streets.

TCPs will be prepared for roadway widening, raised median, and all utility work. The provision of adequate walkways during the construction phase to accommodate significant lincoln Elementary pedestrian traffic will be paramount.

The TCPs will be developed in conformance with the 2014 CA MUTCD requirements and guidelines. A set of 90% TCPs will be submitted to the City for review and comment. Once comments from the City are received, HMM will revise the plans accordingly and prepare a set of 100% Teps for final City review. Traffic control plans will be prepared at 1 "=40' scale.

Task 15 -landscape / Irrigation
A Landscape Concept Plan will be prepared for the proposed median islands and parkway landscape along the project limits.

NUVIS will work dosely with the APP engineering staff and the City to develop alternatives that are consistent with the City's approved tree list and adjacent projects on lincoln Avenue. During the planning we will look for opportunities to:
• Maximize low water shrub areas along fence lines, passive areas, and walkways. • Strategically place trees to reduce heat-island effects, cool parking lots, provide shading for transit areas, and promote health and well ness.
• Utilize current research tools like U( Davis' Water Use Classifications of Landscape Species (WUCOLS IV) to select plants which require low water amounts of water for the particular project site and the California Native Plant Link Exchange (CNPLX) to select appropriate native species for the bio-region. • Specify California native plants to create water-efficient wildlife habitat where appropriate. • Adhere to the latest State, Local, and District water use guidelines like AB1881 and State reduction mandates like the Governor's Executive Order in effect as of June 1, 2015. • Group plants into water use zones to respond to specific microclimates and maximize efficient use of irrigation water. ---- ---- --.---

o Develop detailed water use caleulations including evapotranspiration rates, irrigation efficiencies, plant types, density of plantings, and irrigation application methods.
o Specify weather-based irrigation controllers and irrigation systems which can respond to daily changes in the site's microclimate and adjust run times accordingly.
o Use muleh (wood/bark or rock/gravel) to reduce evapotranspiration and improve soil conditions. o Incorporation, wherever possible, ofbio-swales, bio-retention areas, rain gardens, water re-use systems, cisterns, and other water saving innovative best management practices. o Consider throughout the design process of long-term maintenance requirements for the project site; adjustments made based upon client communication and available maintenance resources.

After review and comment from the Client and design team, the landscape concept plan will be refined into one (1) final conceptual pictorial graphic in colored presentation quality format at an appropriate scale for viewing from a distance. The graphic exhibit will be accompanied by a plant palette containing digital imagery as well as one (1) computer generated before/after. We believe these additional graphic exhibits help to further convey the design intent.

Upon approval of the landscape Concept Plan, final PS&E will be developed for the public landscape and irrigation improvements as well as for the impacted private property landscaping and irrigation. Parkway and median planting plans will be prepared based on the construction plan layout showing plant species, layout, container sizes and quantities, and paving materials, colors and textures. Irrigation caleulations will be prepared based on the planting design using water budgets established by the City. Irrigation equipment will be based on City requirements.

Task 16 - Off-site Improvements
Although the final alignment is not known at this time, we are anticipating Significant off-site improvement reconstruction, which will be identified and evaluated on a case-by-case basis, including, but not limited to the following:
o low height retaining curbs
o landscaping and irrigation relocation
o Private walkways
o Parkway culvert and drainage system relocation
o Wall/fence/pilaster reconstruction
o Backflow preventer relocation
o Parking lot reconfiguration
o Electrical equipment relocation
• Private sign relocation
(optiona/) Building Modification. If building overhang and fa~ade removal are deemed necessary, the APP team will prepare the necessary modifications to the roof overhang and building fa~ade. We have assumed that the building walls will not be impacted. A more specific scope of work will be provided after the extent of work is known but an allowance is provided in the fee estimate.

Task 17 - Drainage
According to information gathered during the proposal preparation and field-verified, there is an existing 42"-63" storm drain in la Palma Avenue that flows westerly with several lateral pipes from the adjacent curb and from the side streets. There are a number of catch basins that will be impacted by the roadway widening.

APP will prepare a hydrology map and hydrology and hydraulic caleulations for the area tributary to the relocated catch basins.

The size of the catch basins and corresponding catch basin laterals will be evaluated. Based on the results of the hydrologic and 20 I P a ge

hydraulic analyses, the appropriate catch basins and connector pipes required will be designed. Off-site drainage relocations will be shown on the off-site improvement plans.

Twenty-scale storm drain plans will be prepared for the projects based on the final approved hydrologic and hydraulic calculations and approved alignment. The plans will include plan and profile drawings, necessary catch basin details, and details required to join the existing facilities.

In September 2009, the State Water Resources Control Board adopted Order No. 2009-0009-DWQ (Waste Discharge Requirements for Discharges of Storm Water Runoff Associate with Activities and land Disturbance Activities). Effective July 1, 2010 all dischargers (construction sites where calculated soil disturbance total 1 acre of more) are required to obtain coverage and comply with this Construction General Permit (CGP). As part ofthe requirements of the state CGP, which covers most local agencies NPDES permits issued by the California Regional Quality Control Board, the Santa Ana Region requires a site specific Storm Water Pollution Prevention Plan (SWPPP) Permit. As of September 2011, the person preparing the plan shall be a Qualified SWPPP Developer (QSD) as defined by the CPG. These new requirements place the filing of the Notice of Intent, the development of the SWPPP and enforcement of compliance to the approved SWPPP on the Owner; as well as any resulting fines from the Contractor's failure to comply with the approved SWPPP. The APP Team is very familiar with the requirements of the State's Construction General Permit and has been preparing SWPPP under these new guidelines since they went into effect.

We have several certified Qualified SWPPP Developers in-house to assist the City in preparation of the necessary documents.

We will prepare, on the behalf of the City, the initial SWPPP for the construction of the project. This is important because no construction activity can be allowed until the City receives the tracking number from the SWRCB.

Due to the planned widening associated with the Project more than 5,000 square feet of impervious area will be added. The Santa Ana Regional Water Quality Control Board requires any project adding 5,000 square feet of impervious area to prepare and implement a Water Quality Management Plan (WQMP) to reduce stormwater pollutants. The project team is up-to-date on the recent changes to the model WQMP and has successfully implemented the new requirements on several projects throughout Orange County. The Project Team will use this project experience and lessons learned to develop the WQMP up front in the project design phase as potential best management practices will be intrinsically tied to the final design solution.

Potential low impact development BMPs will most-probably include tree box filters to treat the 85th percentile storm flows prior to release to the existing storm drain system.

Task 18 - Permits
We have assumed the Contractor will prepare a 5WPPP for the proposed improvements, as well as any permitting required for hazardous material work.

Task 19 - Plans / Specifications / Cost Estimate
Plans and cost estimates will be prepared and submitted to the City for review at the 50 percent (50%) and 90 percent (90%) level. Special provisions will be prepared at the 90% level based on the Standard Specifications for Public Works Construction and the City's boilerplate, supplemented with Caltrans specifications. Pertinent collected utility data, pothole results and geotechnical information will also be provided to the C~y .

Based on feedback from the review and continual input from the City staff, final construction plans will be prepared.

Final plans, specificatiOns and cost estimates will be prepared and submitted to the City after all City comments have been addressed. All deliverables including mylars, master set of specifications, and all digital files will also be provided to the City.

Cost estimates will be prepared and submitted to the City for review at the 50 percent (50%), 90 percent (90%) and 100 percent (100%) levels. APP will provide the City with a diskette of our cost estimate spreadsheet, which can be added to by the City in preparing a bid summary after the Contractor's bids are received. A Value Engineering analysis will be conducted for the proposed project to identify potential cost-saving measures. ---_._---

All plans will be prepared in AutoCAD. Digital files and one set of original my lars shall be provided to the City upon final approval. All submittals will be in Microsoft Word format.

Task 20 - 50% Design Workshop
The APP Team will prepare and present a Power Point presentation to discuss the project background, scope of work, technical information (alignment, traffic, utilities, private property impacts), schedule, and construction costs.

Task 21 - Project Coordination
APP will set up and attend an initial/kickoff meeting with the City to introduce project team members, formalize project communication, discuss project schedule, review scope of work, and request available data, reports, documents, and plans from the City that are relevant to the Project. An additional purpose of the meeting will be to review the project and traffic analysis criteria and identify pertinent traffic issues and concerns.

APP's Project Manager has been managing roadway widening projects since the 1990s for the City of Anaheim and understands the importance of communication with the City. We envision numerous meetings with the City through the duration of the project. APP's Project Manager will communicate regularly with the City's Project Manager regarding progress, potential issues and schedule. In addition, APP will prepare and submit monthly progress reports to the City.

Two public meetings shall be scheduled to help the City further understand community concerns and support for the project. A Public Meeting in an Open House format will be held at a location determined by the City. APP team members will plan and facilitate the meeting, provide exhibits, handouts/fact sheets, wireless headsets and refre5hments, and provide staff for the sign-in table, including an English-Spanish bilingual staff member. APP's Project Manager and other appropriate technical staff will attend this meeting. ECORP will translate the public meeting notices/fact sheets from English to Spanish. The translated text will go through ECORP's Quality Assurance/Quality Control program to ensure proper translation and readability.

APP staff recently conducted a series of contentious public outreach meetings in conjunction with the Richmond Street Widening Project. We were able to successfully obtain consensus through a series of subcommittee and public meetings despite dealing with a varied group comprised of reSidents, businesses and politicians.

Up to four additional public meetings could be requested by the City and are included in our scope of work as necessary. - - ---------------

Task 22 - Meetings
Regularly scheduled meetings are a key to keeping projects on schedule, avoiding pitfalls, and keeping the lines of communication open with the City so that surprises do not occur during the final stages of the project. We have assumed attending one status meeting per month.

Task 23 - Project Management / QA / QC
The Project Management task is directed at the organization, planning and control of the project team. Activities include scheduling and monitoring of the project team, cost control and an active quality control program. Quality Control will be an ongoing task throughout the duration of the project. The initial effort will be in the development of a project specific checklist that will establish specific tasks that need to be accomplished, the estimated completion date and a signature block for the Project Manager to sign that the task has been completed and reviewed. The Quality Control Officer for the project ensures that all checklist items are addressed by the Project Manager. In addition to the Quality (ontrol by the Project Manager, a Quality (ontrol committee comprised of at least two registered engineers that are not involved in the project will review the plans. The review committee will do a technical evaluation of the plans, cost estimate and the project specifications. The review comments are transmitted to the Project Manager during a subsequent meeting.

Task 24 - Bid Phase
The APP Team will provide ongoing support as necessary during the bid phase for answering questions/clarifications from the Contractor and for any revisions due to unforeseen conditions.

Task 25 - (onstruction Phase
The APP Team will provide ongoing support during the bid/construction phase for any questions/clarifications from the (on tractor and for any revisions due to unforeseen conditions, construction observation and preparation of record drawings.
