Be Well will work closely with the City of Anaheim to ensure a high-caliber data process, building on existing systems. Our data capture will include the following:

• Types of service provided and outcomes including:

BE WELL OC Proposal for Mobile Response Team | 5
• Number of unique individuals served (also, high utilizers and those who refuse service) • Housing Status (housed vs. unhoused, and process metrics moving clients toward housing solutions)
• ROI – First responder savings in hours
• Total call volume and demographics

This Year 2 proposal contemplates a three-van model operating on a 12-hour module with staggered shifts for a total of 14 hours of coverage.
The preferred vehicle is a Ford Transit Extended Length with High Roof to accommodate the various interventions and transportation needs while in service in the community. Two of these vans will be active in overlapping schedules in the community. A third, van will be placed in service, specifically supporting the ACCESS program to accommodate transportation needs to and from the court and facility-based care.
Anaheim’s ACCESS Program:
ACCESS (Anaheim Collaborative Court – Evaluating Strategies and Solutions) is a joint effort by the City of Anaheim, the Orange County Superior Court, and the Public Defender’s Office to connect our Courts to our Community to support the stability and enhance the recovery of justice-involved individuals who are willing to engage in treatment. ACCESS provides pathways for justice-involved individuals suffering from homelessness, mental health, and/or substance use disorders to be diverted from the traditional criminal justice system and into community-based treatment, with the Court acting as an accountability partner for change. ACCESS utilizes a multi-disciplinary team and validated assessment tools to create IDPs (Individualized Diversion Plans) based on a person’s unique needs.
Testimonials:
Success Story #1:
On May 6, 2023, the HOPE Mobile Response Team in Anaheim was dispatched to a call regarding a juvenile in crisis. This juvenile has a history of substance use disorder and a diagnosis of bipolar, ADHD, and depression. The juvenile threatened to run away at the scene, and there was immense tension between the juvenile and her adoptive parents. Initially, the juvenile was not open to speaking. Still, through crisis intervention de-escalation techniques, the juvenile began to open up to our Crisis Intervention Specialists (CIS) and discuss why she was upset.
The CIS was able to sit with the family and facilitate an open discussion which aided in diminishing the tension and allowed each member to express their feelings. At the scene, the CIS could also review the resources the family was already connected to, such as therapy, and determine if the family needed additional help. In a follow-up, the HOPE Navigator spoke with the juvenile's mother, who expressed immense gratitude for having the HOPE team dispatched to her house. She stated that she found our CIS “very helpful” and was excited about this service being available in Anaheim. She stated she had heard about our services before but had not utilized our team until this incident. She said her interactions with our team were cheerful and helpful to her family.
BE WELL OC Proposal for Mobile Response Team | 6
The HOPE Navigator learned that the mother was interested in a substance use disorder support group for her juvenile as she already has an extensive history of attending residential treatment centers in California and outside the state. The HOPE Navigator provided the mother with resources to Ala-teen and The Purpose of Recovery for additional support. The HOPE Navigator will continue to follow up with the family to ensure resource linkage and provide additional resources as needed.
Success Story #2:
On April 29, 2023, the HOPE Navigator received a referral for a newly evicted client staying at a motel in Anaheim. The HOPE Navigator met with the client and learned more about the client’s situation. The client shared that she had been evicted in February and was receiving help from her family to obtain housing. However, her father stated that she only had a week left in the motel before running out of money and reaching street-level homelessness. The client also shared that she struggled with mental health problems and received SSI due to her inability to work. The client stated that she was already linked to mental health services through her insurance, but her immediate need was obtaining housing.
The HOPE Navigator also learned that the client had been residing in Fullerton for 6 to 7 years.
With her limited income and being unhoused in Anaheim, the HOPE Navigator contacted the Community Care Response Team (CCRT). The CCRT case manager was able to walk the HOPE Navigator through obtaining emergency shelter at the Salvation Army and Bridges at Kraemer. The CCRT case manager suggested that because the client has a recent eviction and ties to the city of Fullerton, the HOPE Navigator contacted the Homeless Liaison Officer (HLO) in Fullerton to see if they could assist. The HLO team in Fullerton was quick to respond and was able to aid the client in finding a bed at Illumination Foundation. In addition, the HLO team contacted the client and offered to pick her up in Anaheim and drop her belongings off at Illumination Foundation. The client continues to be linked to Illumination Foundation, where she receives assistance and information regarding supportive housing services.
Thank you for the opportunity to submit this Year 2 proposal. It is an honor working with the City of Anaheim, and we look forward to your response.
