COA MA496234 Integrated Technology Contract #1339
Integrated Technology and the City of Anaheim are hereby in agreement to amend the City of Anaheim's 5 Star Premium Service Agreement for Labor and/or Services Agreement. Contract Number 1339 COA MA496234 for the Nortel/Avaya Systems. This addendum is to extend the existing contract for 12 months. Upon completion of the 12 month term, the contract will then become month to month.
Monthly Maintenance Support Fee based upon a 1 Year Agreement: $8,994.16 Agreement terms:
Start Date: January 1, 2025
End Date: December 31, 2025
Payment Terms: To be Paid on a Monthly Basis
New Month to Month Start Date: January 1st 2026
New Month to Month Rate at the completion of the new 12 month term: $8,994.16 Payment Terms: Net 30 days (No change)
ALL OTHER TERMS OF CONTRACT TO REMAIN THE SAME.
