We, the undersigned contractor, have given careful consideration to the change proposed and hereby agree, if this proposal is approved, that we will provide all equipment, furnish all materials, except as may otherwise be noted above, and perform all services necessary for the work above specified, and will accept as full payment therefore the prices shown above.

Description of work to be done, estimate of quantities, and prices to be paid. Segregate between additional work of contract price, agreed price and force account. CHANGE ORDER NO.
In accordance with section 4-1.05, "Changes and Extra Work", of the Standard Specifications, provide labor, equipment and materials as necessary to perform the work listed below. All work shall be done in accordance with the attached revised plans, Special Provisions, Standard Plans and Specifications and as directed by the Engineer.
Increase in Contract Item at Contract Item Price:
Construct an additional 25' of Concrete Barrier (Type 60 M) at "MI5" alignment from Station 32+95.57 to Station "MI5" 33+20.57, as shown on the sheet 2 of this change order and Reconstruct the anchor block at the new location as shown on the revised sheet of this change order Item No.188- Concrete Barrier (Type 60M); 25 LF @ $82/LF (+1.4%) = +$2,050.00 (+1.4%) Item No.9- Minor Concrete (Minor Structure); 2 CY @ $2,900.00/CY (+28.5%) = +$5,800.00 (+28.5%) TOTAL = $7,850.00
Change at No Cost:
Change Anchor Block to Type Mod1 shown on sheet 2 of this change order.
Extra Work at Forced Account:
Remove the anchor block constructed and the posts for the WB-31 per plans to install at the locations shown the revised sheets of this change order, and miscellaneous work needed to peform this work not covered by any of the items = $10,000.00 For this work, the contractor will be compensated for the actual costs. Contractor will submit invoices and extra work tickets in accordance with Section 9-1.04C of the Standard Specifications.
