We, the undersigned contractor, have given careful consideration to the change proposed and hereby agree, if this proposal is approved, that we will provide all equipment, furnish all materials, except as may otherwise be noted above, and perform all services necessary for the work above specified, and will accept as full payment therefore the prices shown above.

Description of work to be done, estimate of quantities, and prices to be paid. Segregate between additional work of contract price, agreed price and force account. CHANGE ORDER NO.
As authorized by the Engineer, and in accordance with section 4-1.05 "Changes and Extra Work" and as requested by the Contractor, from Station 81+19.00, "MI6" alignment, to Station 691+03.79, "SR99" alignment, stripe the Southbound off ramp at Ming Ave as shown on sheet two of this change order. After removal of the falsework system for Frame 1 of the N99-W58 connector for the Mainline project (Contract No. 19-069) the Contractor shall stripe the offramp as per the Pavement Delineation Plans.

There shall be no consideration of a time adjustment for the work on this change order. The time adjustment for this change order shall be 0 (zero) working days.
