We, the undersigned contractor, have given careful consideration to the change proposed and hereby agree, if this proposal is approved, that we will provide all equipment, furnish all materials, except as may otherwise be noted above, and perform all services necessary for the work above specified, and will accept as full payment therefore the prices shown above.

Description of work to be done, estimate of quantities, and prices to be paid. Segregate between additional work of contract price, agreed price and force account. Unless otherwise stated, rates for rental of equipment cover only such time as equipment is actually used and no allowance will be made for idle time. 18-176 96

CHANGE ORDER NO.
As authorized by the Engineer, and in accordance with section 4-1.05 "Changes and Extra Work", provide additional funds for change order No. 60 to restripe highway 58, westbound, as shown on sheets 2 and 3 of this change order, and remove and replace the Wide Flange Beam at Station 102+69 "CN5" alignment installed under change order No. 3, and remove the concrete paving from Station 102+39 to Station 102+69, No. 3 lane, "CN5" alignment and replace it to the grades shown on sheet 4 of this change order.
EXTRA WORK AT FORCE ACCOUNT:
a) Provide traffic control to stripe the CN5 alignment as shown on sheet 2 of this change order. b) From Station 102+39 to 102+69, No. 3 lane, "CN5" alignment, and as directed by the Engineer, remove and replace the existing concrete paving and the wide flange beam.
c) Replace the concrete paving removed under "b" above to the elevations shown on sheet 4 of this change order or as directed by the Engineer. d) At Station 102+69, "CN5" alignment, as shown on sheet 3 of contract change order No. 3, and as per Standard Plan P32A, install a new wide flange beam. The Contractor shall procure a 30' section of Wide Flange Beam and cut to fit in the field. e) Compensate the Contractor for the additional mobilizations for subcontractors.
Extra Work at Force Account: $40,000.00
