We, the undersigned contractor, have given careful consideration to the change proposed and hereby agree, if this proposal is approved, that we will provide all equipment, furnish all materials, except as may otherwise be noted above, and perform all services necessary for the work above specified, and will accept as full payment therefore the prices shown above.

As authorized by the Engineer, and in accordance with section 4-1.05 "Changes and Extra Work" of the Standard Specifications, provide all labor, equipment, tools, and materials to place 195 additional cubic yards of concrete for the slope paving at the locations shown on the plans.
Bid Item No. 153, Slope Paving (Concrete): 201 CY (+25.8%) @ $1,500.00/CY = $301,500.00 (+25.8%) In accordance with Section 9-1.06, “Changed Quantity Payment Adjustments,” of the Standard Specifications, you will receive no additional payment for the change in quantity on the following items:
