1. Why is your organization requesting an in-kind donation? We are requesting field fee funds to cover and lower out of pocket costs for our Barstow families. While our coaches and board members are all volunteer, there are still many expenses for youth to play competitive sports. Our organization's objective is to provide a zero out of pocket cost to play competitive soccer. In comparison, other local soccer clubs charge from $1200 to upwards of $3500 per year per player. Our players are learning, performing, and winning despite not paying.

2. How will this in-kind donation benefit your organization and the youth population of Barstow?
The allotment of funds to cover fields fees will help our non profit organization to continue to provide a competitive soccer outlet for Barstow youths seeking higher level competition and development. Our organization is now the 2nd largest club in Coast Soccer League under CAL South (our governing body), and we continue to grow because we are filling a need for our community. As always, we never charge overhead or coaching fees. This donation will provide the necessary place and equipment to allow our youth players to practice more effectively and productively.

With this application, please attach the following:

1. A detailed description stating the total cost of the activity or event, all sources and amounts of funding, and all expenses;

2. A description of the organization's or group's purpose or objective;

3. A five-year operating plan outlining the goals and objectives for their organization;

4. A copy of the organization's non-profit determination.

By signing below, I declare that I have read the Youth Community Promotional Fund Policy and agree to all its terms and conditions. I agree that there is a formal process in place to conduct background checks on all employees and volunteers who work with persons under the age of 18 years and senior citizens. Dear Applicant:

We're pleased to tell you we determined you're exempt from federal income tax under Internal Revenue Code (IRC) Section 501(c)(3). Donors can deduct contributions they make to you under IRC Section 170. You're also qualified to receive tax deductible bequests, devises, transfers or gifts under Section 2055, 2106, or 2522. This letter could help resolve questions on your exempt status. Please keep it for your records.

Organizations exempt under IRC Section 501(c)(3) are further classified as either public charities or private foundations. We determined you're a public charity under the IRC Section listed at the top of this letter.

If we indicated at the top of this letter that you're required to file Form 990/990-EZ/990-N, our records show you're required to file an annual information return (Form 990 or Form 990-EZ) or electronic notice (Form 990-N, the e-Postcard). If you don't file a required return or notice for three consecutive years, your exempt status will be automatically revoked.

If we indicated at the top of this letter that an addendum applies, the enclosed addendum is an integral part of this letter.

For important information about your responsibilities as a tax-exempt organization, go to www.irs.gov/charities.

Enter "4221-PC" in the search bar to view Publication 4221-PC, Compliance Guide for 501(c)(3) Public Charities, which describes your recordkeeping, reporting, and disclosure requirements.

Sincerely,
stephore a. marten
