RECOMMENDATION
Staff recommends that the City Council approve the following items to further improve the quality and efficiency of the Police Department’s medical services through a threeyear pilot program:

1. Amendment No. 3 to an Amendment No. 3 to an Agreement Between the City of Beverly Hills and Marc R. Cohen, MD, Professional Corporation to expand the Fire Department Medical Director's scope to include Police Department medical services. This amendment adds $273,482.17 for the term from September 1, 2025, through June 30, 2028, increasing the total contract value to $907,191.97.

2. Authorize the City Manager to execute an agreement with a contract EMS Nurse Educator (Registered Nurse) to staff the new centralized medical programs for a total amount not to exceed $620,817.60 for the term from September 1, 2025, through June 30, 2028.

INTRODUCTION
The Beverly Hills Police Department (BHPD) manages several essential medical service lines, including 911 Emergency Medical Dispatch (EMD), the Naloxone administration program, and medical screenings for individuals in the City jail.

To improve the oversight of all police-related medical functions, the Fire Department proposes a three-year pilot program to centralize these services under its existing EMS Division. This approach leverages the Fire Department's established medical leadership and quality improvement infrastructure. The program will be managed by expanding the role of the Fire Department’s existing Medical Director and adding an additional EMS Nurse Educator.

This strategic consolidation is designed to be primarily cost-neutral, as it will be funded almost entirely by reallocating existing Police Department funds budgeted for these services and by utilizing restricted Opioid Settlement grant funds already received, or to be received, by the City.

BACKGROUND
The City’s Police Department provides medical services that are critical to public safety but require specialized oversight to ensure regulatory compliance and adherence to community standards of care.
• Emergency Medical Dispatch (EMD): BHPD dispatchers provide pre-arrival medical instructions to 911 callers, a function that L.A. County LEMSA defines as a medical directive requiring physician oversight.
•
• Naloxone Program: BHPD officers are equipped with Naloxone to reverse opioid overdoses. As this involves medication administration, the program requires clinical oversight, quality assurance, and recurring training for officers, which is not currently formalized.
•
• Jail Medical Services: The Police Department is responsible for pre-booking medical screenings and DUI-related blood draws. These services are currently Meeting Date: August 19, 2025

Page 3 of 6 ends

outsourced to a third-party vendor and require medical oversight to ensure proper protocols are followed and acutely ill individuals receive appropriate care.

The Fire Department's EMS Division already possesses the expertise, leadership, and quality improvement frameworks necessary to manage these functions effectively and bring the City into full compliance.

DISCUSSION
This proposal consolidates the three police medical service lines under the Fire Department's EMS Division, leveraging existing personnel and proven quality management systems to create a more efficient and compliant program. This will be accomplished by expanding the duties of the Fire Department's Medical Director, Dr.

Marc Cohen, and adding one contract EMS Nurse Educator.

Ensuring Compliance and Enhancing Services
The expanded Medical Director and new EMS Nurse Educator will provide the necessary oversight and program management to fully satisfy LEMSA requirements and enhance the other medical programs.

The Medical Director's expanded duties will include:
● Update current EMD protocols to LEMSA standards (LEMSA Reference Number 227.1),
● Assist in the development of a comprehensive quality assurance and improvement program, as defined under LEMSA Reference Number 620, ● Assist in the implementation of LEMSA evidence-based initiatives to improve patient outcomes,
● Assist in the ongoing education and training for call takers, and ● Provide direct consultation on complex cases such as Mass Casualty Incidents (MCI) or special events.

In tandem with expanding the BHFD’s Medical Director to oversee the EMD program, the addition of one EMS Registered Nurse Educator will execute the deliverables. The EMS Nurse Educator will provide the following new EMD related services:
• Conduct a comprehensive review of dispatch policies and practices to assess alignment with LEMSA Reference Nos. 227 & and Pre-Arrival Instructions Reference No. 227.1,
• Evaluate and revise pre-arrival instruction protocols (32) to align with LEMSA 227.1.
• Create a quality assurance and improvement plan for EMD delivery, including measurable and reportable metrics,
• Attend LEMSA EMD related meetings, serving as an advocate for BHPD, as it relates to EMD delivery, as well as keeping current to Regional, State, and Federal EMD related changes,
• Review EMD related calls, focusing on high-risk and vulnerable populations, coordinating with BHFD response and care,
• Develop a continual education plan for BHPD Dispatchers and provide said education in regular intervals.

Meeting Date: August 19, 2025

Page 4 of 6 ends

The integration of both the BHFD Medical Director and the EMS Nurse Educator as well as the implementation of the aforesaid duties are designed to enhance the continuum of care from 911 activation to post Fire / EMS response.

In addition to attaching BHFD’s Medical Director and adding an EMS Nurse Educator to BHPD’s EMD program, both will also be integrated into BHPD’s Naloxone program.

Together, both the BHFD Medical Director and the EMS Nurse Educator will provide quality assurance and improvement measures, continual education, and medical control to the program.

Lastly, both BHFD’s Medical Director and newly added EMS Nurse Educator will be an integral part in BHPD’s Jail Medical Services, both from an administrative and operational position. Jail Medical Services administrative duties include, but are not limited to:
• Development and review of medical screening protocols for persons in custody, • Periodic continual education related to identifying acutely ill conditions, • Real time case consultations,
• Clear to Book practice management, and
• Periodic audits performed by Local, State and Federal entities (e.g. California Department of Health Care Services).

Forensic Phlebotomy & Body Fluid Collection (service administrative duties include, but are not limited to:
• Medical oversight
• Collection training,
• Collection credentialing,
• Procedures to support phlebotomy draws and collections, • Quality assurance reviews, and
• Medicolegal consultation.

Operationally, during BHFD’s Advanced Provider Response Unit (AP1) in-service hours of 0700 to 1900, Jail Medical Services will shift from BHPD’s current third-party vendor to BHFD’s Nurse Practitioner service (AP1).

For hours that AP1 is not in service, BHPD will enter into an Agreement with a third-party vendor to provide Jail Medical Services (Pre-booking medical screening evaluations and DUI blood draws). The Naloxone Program and the EMD program will be administered by BHFD.

Performance and Success Metrics
The success of this three-year pilot program will be evaluated using key performance metrics modeled after the Fire Department's comprehensive EMS Quality Improvement Plan .

These metrics will include:

EMD Program: Full compliance with LEMSA pre-arrival instruction protocols (Ref. No. 227.1) and quality improvement standards (Ref. No. 620). • Jail Services: Tracking and reporting on patient care outcomes to ensure quality and prevent negative incidents.
• Naloxone Program: Documenting the application of protocols, completion of continual education, and quality improvement measures.

Meeting Date: August 19, 2025

Page 5 of 6 ends

• Compliance Tracking: Time and effort spent by the Medical Director and Nurse Educator on opioid-related activities will be tracked to ensure compliance with the terms of the Opioid Settlement Funds.

At the conclusion of the pilot program in FY 2027-28, the City will evaluate the program's effectiveness and cost to determine future steps.

EMS Nurse Educator services are considered professional services and as such as exempt from the City’s bidding requirements pursuant to Beverly Hills Municipal Code § 3-3-112 (G). However, in order to exercise due diligence, the Fire Department conducted an informal solicitation process to identify a highly-qualified EMS nurse educator for contract award,

FISCAL IMPACT
This proposal has a minimal net fiscal impact on the City. The total new expenses of $894,299.77 over the three-year pilot period are almost entirely offset by the transfer of existing budgeted funds from the Police Department and the use of restricted opiate settlement grant funds. The total net cost to the City over three years is $17,540.73, which can be absorbed by the Fire Department’s existing budget.
