Task 3 - Groundwater Well Sampling Services
A total of 19 wells at 8 distinct locations shall be sampled for this Project at the frequency shown on Table 2. The Consultant shall obtain groundwater samples from these wells using details presented In Table 1 - Well Location Information and Table 2 - Well Sampling Details. Groundwater samples collected from these wells shall be delivered to the partner laboratory for sample analyses as shown on Table 3.

Notably, three locations shown on Table 1 consist of triple-nested wells including MW-1 (MW-1/1, MW- 1/2, and MW-1/3), MW-2 (MW-2/1, MW-2/2, and MW-2/3), and MW-3 (MW-3/1, MW-3/2, and MW-3/3).

Each of these triple-nested wells are contained In a single well vault. A total of four of these nested well casings (MW-1/2, MW-1/3, MW-2/2;- MW-2/3. MW-3/2 and MW-3/3) contain a 2-inch diameter schedule (SCH) 40 polyvinyl chloride (PVC) well casing liner inside a 3-inch diameter SCH 80 well casing to depths shown on Table 2. The Consultant shall take appropriate precautions during pump, tubing, and sampling equipment deployment in these wells to ensure that the liner casings are secure and not damaged during sampling activities.

Additional groundwater sampling details are as follows:
• The Consultant shall download pressure transducer water level data from 14 wells as described in Task 2.
• The Consultant shall measure water levels (i.e., gauge) at 19 wells seml-annually in April and October and 10 wells quarterly in January, April, July and October of each year per Table 2. • After measuring water levels and prior to collecting samples, each well shall be purged to remove water that has been standing within the well screen and casing that may not be chemically representative of formation water. The Consultant shall purge and sample 19 wells semi-annually in April and October of each year and 10 wells quarterly in January, April, July and October of each year per Table 2. Well purging shall consist of one well volume removal at maximum purge rates shown on Table 2. Purge water removed from each will be discharged to an on-site sewer connection, containment, or discharge point provide by the City. • The Consultant shall measure field water quality parameters' during well purging activities including electrical conductivity, temperature, and pH as shown in Table 2. This data will be recorded on field forms that will be provided to the City at the end of each sampling event. • Upon completion of groundwater purging at each well, the Consultant shall collect groundwater analytical samples in laboratory provided sample containers for analysis of total dissolved solids, chloride, sulfate, and manganese using the analytical methods shown on Table 3. • Filed duplicate samples shall be collected at 10% of total samples during each event. Trip blanks, field blank, or temperature blank are not required for this project. • Additional requirements and qualifications for this task are further described in the Minimum Requirements and Qualifications of this RFP.

Purge Water Disposal
Groundwater sampling purge water will be discharged or transported as stated below:
• Off-site Transport and Disposal by Consultant - Groundwater sampling purge water from wells 20D01S (MW-1), 20D02S (MW-2), 2GD03S (MW-3), 20DG4S (MW-4), and 2GD05S (MW-5) will be stored in a temporary water tank supplied by the Consultant and then transported off-site for disposal by the Consultant. Estimated purge volumes for these 5 wells are shown on Table 2.

The Consultant shall be responsible for storing water and any coordination, lab analysis, or requirements of the selected waste disposal facility. The City will sign waste manifests supplied by the Consultant and waste disposal facility.

• Discharge to Sewer - Purge water from wells MW-1/1, MW-1/2, MW-1/3, MW-2/1, MW-2/2, MW-2/3, MW-3/1, MW-3/2, and MWt3/3 will be discharged to a nearby sewer under existing City of Camarillo National Pollutant Discharge Elimination System discharge permit. Purge water volume estimates for these wells are shown on Table 2. Assume a maximum of 50 feet from well to nearby sewer at each well.
• No Purge Water - Wells 34G3, 19E1, 19M6, 19B1, and 19B2 are active production wells.

Samples will be collected from sampling ports at the well head.

Deliverables:
1) Groundwater sampling field forms showing, but not limited to, total volume purged, record of field water quality measurements, purge rates, and sampling times. 2) Field chain-of-custody forms provided to the laboratory after groundwater sampling activities.

There must be no cross- liability exclusion for claims or suits by one insured against another. The insurance must include a waiver of subrogation applicable to the insurance or self-insurance, a primary and non-contributory endorsement, and an additional insured endorsement, ail in favor of the City, Its officers, employees and agents, and volunteers. Such insurance must include a waiver of subrogation endorsement in favor of City, its officers, employees, agents, and volunteers.

6. Professional Liability (Errors & Omissions) Insurance. The professional liability insurance must cover the services to be performed under this Agreement. The coverage must be provided on a "claims made" basis. Consultant must maintain continuous coverage through a period not less than three years after the completion of the services required under this Agreement.

7. Umbrella or Excess Liability Insurance, if an excess or umbrella liability policy is used to meet minimum limit requirements, the insurance must provide coverage at least as broad as specified for the underlying coverages. Any such coverage provided under an umbrella or excess ilabiiity policy must include a "drop-down provision" requiring the policy to respond in the event that any primary insurance that would otherwise have applied proves to be uncollectable in whole or in part for any reason. Coverage must be provided on a "pay-on-behalf basis, with defense costs payable in Form Approved January 2018 C-1
00 Agreement 2020-162
addition to policy iimits. There may be no cross-iiability exclusion precluding coverage for claims or suits by one insured against another. The policy must "follow form" to the underlying primary policy.

9. Certificates of Insurance and Endorsements; Notice of Termination or Changes to Policies.

Prior to commencing any services under this Agreement, Consultant must file with the City certificates of insurance and endorsements evidencing the existence of ail Insurance required by this Agreement, along with such other evidence of Insurance or certified copies of policies as may reasonably be required by City. These certificates of insurance and endorsements must be in a form approved by the City's legal counsel. Consultant must maintain current certificates and endorsements on file with City during the term of this Agreement reflecting the existence of all required insurance. Each of the certificates must expressly provide that no material change in the policy, or termination or cancellation of the required coverage, will be effective except upon 30 days' prior written notice to City by certified mail, retum receipt requested (except for nonpayment for which a 10-day notice is required). The delivery to City of any certificates of Insurance or endorsements that do not comply with the requirements of this Agreement will not waive the City's right to require compliance. Any insurance proceeds available to City in excess of the limits and coverages required by this Agreement, and which is applicable to a given loss, must be made available to City to compensate it for such losses.

12. Required Insurance for Subconsultants/Subcontractors. Consultant agrees to ensure that any subconsultants/subcontractors providing services under this Agreement provide the same minimum Insurance coverage and endorsements required of Consultant. Consultant agrees to review and monitor ail such coverage and assumes responsibility for ensuring that such coverage is provided in conformity with the requirements of this Agreement.

Form Approved January 2018 C-2
CO Agreement 2020-162
