Staff Report
City Council
Item No. 3.O
Meeting Date: July 9, 2025
From: Michael Contreras, Fire Chief
Title: Approval of Agreement for Wellness and Occupational Health Services with Hoag Corporate Health
RECOMMENDATION:

To approve an agreement for services between the City of Cathedral City and Hoag Corporate Health for a two-year term in the amount not to exceed $170,068.00, for the provision of wellness, fitness, and occupational health services for Cathedral City Fire & EMS personnel and authorize the City Manager or his designee to execute all necessary documents.

BACKGROUND:

In September 2024 Cathedral City Fire & EMS was awarded an AFG grant in the amount of $154,607.27 for annual evaluations to cover physical, behavioral and cancer screenings.

Cathedral City Fire & EMS has prioritized firefighter health and wellness as a strategic initiative aligned with NFPA standards and industry best practices. In alignment with NFPA 1500 (Occupational Safety and Health Program) and NFPA 1582 (Comprehensive Occupational Medical Program for Fire Departments), the department initiated a competitive evaluation of vendors to provide annual physical, cancer screening, behavioral health services, and advanced fitness-for-duty evaluations. This award requires 10% or $15,460.73 in matching funds, which the fire department has budgeted.

A committee of three fire department personnel was put together to evaluate 4 providers that submitted RFP's to include:

Vincere Scottsdale, AZ
On Duty Health Redlands, CA
Hoag Corporate Health Newport Beach, CA
Pinnacle Training Systems Fresno, CA
The committee scored the proposals on four (4) criteria, and each category was scored on a 1 - 5 scale. Two providers were selected for site visits Hoag Corporate Health and On Duty Health. Following the site visits, a comprehensive comparison of vendor services and pricing, Hoag Corporate Health was selected based on its advanced offerings, proven healthcare partnerships, and ability to provide cutting-edge cancer screening, PFAS testing, mental health support, and firefighter-specific health monitoring.

DISCUSSION:

The proposed agreement with Hoag Corporate Health includes three major service components:

1. Cancer Screening Physicals
Includes PFAS testing, Trucheck early-stage cancer screening, and NFPA 1582- compliant exams. These screenings target occupational exposures unique to firefighting and support early detection and intervention.

2. Behavioral Health Services
Includes mental health screenings, one-on-one consultations, and critical incident stress debriefings. These services align with NFPA 1500 and are integrated with the City’s EAP for seamless support.

3. Annual Physicals & Fitness Evaluations
Includes comprehensive lab panels, firefighter heart health assessments, infectious disease testing, and cardiopulmonary exercise testing (CPET). These evaluations ensure personnel are fit for duty and receive preventative care.

The contract is for a not-to-exceed amount of $170,068.00 and includes both on-site and off- site service delivery. Compared to other vendors, Hoag offered the most advanced cancer screening and cardiac testing protocols while providing a competitive price structure and only requires the fire department to only pay for the physicals they receive.

The agreement will be managed by the Fire Chief or designee, with billing and reporting handled monthly as per the contract's terms. Individual health data will remain confidential per HIPAA, with only aggregate compliance data shared with the city.
