City of Commerce Waiver & Release Form
WAIVER OF LIABILITY
I, ___________________________________(Print Legal Name), agree to unconditionally waive and release the City of Commerce, its officers, agents, employees and volunteers (collectively “City”), from and against any claims, costs, liabilities, expenses or judgments, including attorney’s fees and court costs arising out of my choice to be a passenger in a City owned vehicle, that I, my personal representatives, assigns, heirs and next of kin, may have for any loss, damage, or injury to person or property, whether caused by negligence or otherwise of the City and/or the operator of the vehicle. I certify that I am in good physical condition and able to travel in a City owned vehicle. I expressly agree that this Release and Waiver is intended to be as broad and as inclusive as permitted by the laws of the State of California, and that if any portion thereof is held invalid, it is agreed that the remainder shall continue in full force and effect.
