OVERVIEW
HazMat Doc (HMD) has completed a designated Pre-Modeniization/Renovation Asbestos and Lead Survey project at the existing Office Building, located at 10455 Torre Avenue, Cupertino, CA 95014. This work was performed in response to a request by Ms. Ayano Hattori, Project Manager for the City of Cupertino Public Works Department (CoC-PWD), and authorized by Ms. Susan Michael, Capital Improvement Programs Manager for the CoC-PWD.
The purpose of the Survey was to determine if any materials tliat may be impacted, as indicated by Ms. Hattori, as a part of the Modernization/Renovation, would require controls as specified in Title 8, California Code of Regtilations (OSHA), sections 1529 (Asbestos) and 1532.1 (Lead). HazMat Doc personnel visited the Site on February 14 and 15, 2022. Ms. Hattori, our Site Contact for this Srirvey, made all the necessary arrangements for access.
OBSERV ATIONS AND WORK PERFORMED
Tliis Designated Survey is for tlie existing one (l) single-story + mezzanine office building identified as 10455 Torre Avenue, Cupertino, CA 95014. The office building is one (l) main structure, with a small mezzanine extension on the West side. This office building first floor, (both main structure and extension), is comprised of individual offices, and common area restrooms, storage rooms, interior hallways, lobbies, stairways, conference room, ai'id kitchen. Tlie office building mezzanine level is comprised of a large conference room, storage rooms, and three individual attic spaces. There was no interior access to either roof.
The Survey was performed for all accessible materials/surfaces in tlie selected areas, as directed by Ms. Hattori:
o all interior areas for both the main and extension sections of the office building; and o the exteriors and the roof of the west section were surveyed for possible penetration work dtiring renovation.
Asbestos Bulk Sampling
Fifly-one (51) suspect bulk asbestos samples were collected during the Survey. Samples were collected from various locations within and on the above referenced building. The samples were appropriately bagged, labeled and prepared for delivery. All samples were transported and delivered under cliain of custody to EMSL Analytical, Inc., in Las Vegas, NV, for Asbestos Analysis of Bulk Materials via EPA 600/R-93/116 Method using Polarized Light Microscopy (PLM). Original Laboratoiy results are enclosed for review and inclusion with the District's records for this Site.
This Survey is limited to and covers only the accessible materials and surfaces in selected areas, as designated by Ms. Hattori, that corild be impacted as part of the Modernization/Renovation Project. However, materials may be present in inaccessible areas and/or not in the directions from Ms. Hattori as surfaces scheduled to be impacted, and therefore are not included in tliis Report.
Lead by XRF
Non-invasive/non-destructtve readings were collected from suspect materials/surfaces using a NITON X-Ray Fluorescence Spectrometer (XRF) instrument model XLp-300. Paints or coatings found to contain Lead in a concentration equal to or greater than 1.0 micrograms per centimeter squared (mg/cm2) as measured by XRF, are considered Lead-based paints by the Environmental Protection Agency (EPA). Items that are at or exceed this threshold are printed in boldface on the report enclosed for review and inclusion with the District's records for this site.

Hazardous Materials Observations
Additionally, the following known and/or suspect hazardous n'iaterials were observed during the Survey, (or the possible presence of concealed materials in relation to visible fixtures):
* Fire Doors witli suspect core insulation.
* Concealed Thermal System Insulation (TSI) on pipefittings and pipe runs. * Fltiorescent light fixtures, all with suspect Mercury vapor (gas) containing light tubes and suspect PCB and/or non-PCB containing ballasts in each fixture.
HVAC rinit with refrigerant gas.
Tlie asbestos sample results, XRF readings, and liazardous material observations were used to create the Asbestos and Lead Scopes of Work for the proposed Modemization/Renovation Project for the Site. The Scope(s) include(s) materials that would require controls as specified in Title 8, California Code of Regulations (OSHA), sections 1529 (Asbestos) and 1532.1 (Lead).
The Asbestos and Lead Scopes of Work include only areas and materials to be impacted as indicated either on the architectural drawings fiirnished by or designated in directions received from the Project Managers. DRAFT copies of the Asbestos and Lead Scopes of Work are included in Part m of this report for review.
StJMMARY OF RESULTS AND RECOMMENDATIONS
Since the building is currently accessible to general occupants, HazMat Doc did NOT perform a full destructive survey of the property, i.e., orir inspector(s) did not tear into or destroy suspect finishes in order to detertnine what material, if any, might exist in wall/ceiling/roof cavities, below grade, etc. It is however, recommended that an attempt be made to discover potentially "concealed" material(s) prior to constniction activity that might impact those areas.
Asbestos Bulk Sampling
Any friable material having greater than 1% of asbestos fiber content is considered to be a Regulated Asbestos Containing Material (RACM) by EPA. These materials should be handled by a licensed asbestos abatement contractor, prior to any modernization/renovation activity that might disturb these materials. Please note, however, that dishirbance or abatement of any asbestos containing material (even less than 1%) should be performed by trained, certified, licensed and protected personnel who perform this work in accordance with applicable Regulations. If any concealed suspect asbestos containing materials are discovered during further investigations prior to demolition/constniction activities, they should be tested for asbestos content prior to being disturbed.

SUMMARY OF RESULTS AND RECOMMENDATIONS (continued) Lead bv XRF
Seventeen (17) of the readings collected from suspect materials!surfaces by XRF indicated the presence of Lead in excess of 1.00 milligrams per square centimeter (1.00 mg/cm'). Paints or coatings found to contain Lead in a concentration equal to or greater than 1.0 mg/cm' as measured by XRF, are considered Lead-based paints by the EPA. These materials should be handled by a licensed Lead removal contractor, prior to any modemization/renovation activity that might disturb these materials. However, it is important to note that there is Lead present in those surfaces with readings reported as "negative" (i.e. below 1.00 mg/cm2 but above O.00 mg/cm2).
The Occupational Safety and Health Administration (OSHA) states that the disturbance of any Lead containing/coated material(s), regardless of the concentration of Lead, is considered a potential hazard. Any trades that might impact these surfaces must be made aware of the presence of Lead. These materials should be handled by appropriately trained and protected personnel who perform this work in accordance with applicable Regulations.
Lead Notification
As directed by the Califomia Department of Public Health (CDPH) in their Lead Hazard Evaluation - Reporting Requirements Reminder dated June 5, 2006 we have submitted lhe requisite CDPH form 8552 to the Childhood Lead Poisoning Prevention Branch in Richmond, California. A copy of the same CDPH form 8552 is attached to this report for your records.
Please note, this is not a complete survey of asbestos containing materials or of niaterials/sufaces that may be coated with Lead containing paint for this Site. This Survey has been exclusively focused on those accessible materials/surfaces that are in the areas designated to be impacted as a part of the Modemization/Renovation Project by Ms. Hattori.
