Summary of Active Sewer Lines:
1. 1333 Park Ave:
o CL 17 (50.6 ft, Vitrified Clay Pipe)
o CL 18 (69.8 ft, Vitrified Clay Pipe)
2. 1100 Park Ave (Skate Park Restroom):
o Sewer main (42.5 ft, SDR 17 HDPE Polyvinyl Chloride)
Scope of Work
Following the failure of the sewer main air test conducted at the Skate Park Restroom at 1100 Park Ave, the following findings and recommendations have been made:
Findings:
x The existing 4-inch cast iron two-way cleanout access located approximately 10 feet from the restroom at a depth of 4 feet has failed the air test. x A 45-degree fitting located before the mainline connection at a depth of 7 feet was also found to be unable to hold pressure, causing the test failure.
Recommendations:
To comply with EBMUD requirements and ensure the sewer main passes testing:
1. Replace the cast iron two-way cleanout access with a new Cast Iron two-way cleanout.
2. Replace the 45-degree fitting to restore proper functionality and pressure integrity.
3. Perform a new air test to verify compliance with EBMUD requirements.
4. Provide detailed documentation and testing results post-repair. ([KLELW$

o C/O 1 (56.2 ft, Polyvinyl Chloride)
Test Results: PASSED
4. 4321 Salem St (Senior Center):
o C/O (57.8 ft, Vitrified Clay Pipe)
Scope of Work
Following the sewer main air test conducted at 4321 Salem St (Senior Center), the following findings and recommendations have been made:
Findings:
x The section exiting the back of the building to C/O 1 (approximately 23.5 ft in length) could not be tested because C/O 1 is a one-way cleanout access, and there is no building cleanout exiting the back of the building. It appears to be a different type of pipe material. x From the front of C/O 1 (one-way cleanout) to Cleanout Access C/O 7 and all the way to the manhole, the air test holds pressure (partial test passed). x C/O 2 through C/O 7 hold pressure.
x All other sections of the sewer lateral hold pressure and comply with EBMUD requirements.
Recommendations:
To comply with EBMUD requirements and ensure the sewer main passes testing:
1. Replace C/O 1 cast iron one-way cleanout access with a new cast iron two-way cleanout.
2. Add a two-way cleanout access to the branch exiting from the back of the building to C/O 1 and replace the sewer line in between with new SDR 17 using the open trench method for proper functionality and pressure integrity.
3. Perform a new air test to verify compliance with EBMUD requirements.

Test results: FAIL
Following the lower sewer lateral 1 test conducted at 1379 62nd St, the following findings and recommendations have been made:
Findings:
x The existing 4-inch VCP (Vitrified Clay Pipe) sewer lateral has multiple offsets and holds water approximately 10 feet from the cleanout, causing the system to fail.
Recommendations:
To ensure proper functionality and compliance with local regulations:
1. Replace the lower sewer lateral from C/O 1 on the sidewalk to the main city line connection with new SDR 17 pipe using the open trench method.
2. Perform a final air or water test to confirm proper installation and compliance.

Following the partial testing conducted on the CL 4 Upper Sewer Lateral (End of Pipe) at 1379 62nd St, the following findings and recommendations have been made:
Findings:
x The 70-foot cast iron sewer lateral was partially tested. The first 40 feet passed the air test.
x The remaining 30 feet could not be tested due to the lack of a cleanout. x Pipe integrity is good; however, additional testing is required to ensure compliance with EBMUD standards.
Docusign Envelope ID: BE2FF941-8646-4183-8881-6CECD97F01C1 Recommendations:
1. Install a two-way cleanout access at the end of the upper sewer lateral line to facilitate proper testing and future maintenance.
2. Perform an air or water test on the remaining 30 feet of the line to verify compliance with EBMUD requirements.
3. Document testing results and provide final compliance certification.
