Use of a Coordinated Entry System is a requirement for:

• Emergency Solutions Grant (ESG) program recipients
• Other Federal programs providing funding for homeless services, including those provided through the U.S. Department of Veterans Affairs • Other state funding, such as Homeless Housing, Assistance and Prevention (HHAP) and No Place Like Home.

❖ Streamlines, standardizing and coordinating the homeless intake, assessment, and referral processes.
❖ Allows individuals and families who need
assistance for housing to have one door for
assistance and be assessed and referred to the
appropriate intervention.
❖ Prioritizes households to ensure a fair and
equitable process for distributing limited
resources based on need and vulnerability.

❖ Assessment: Conduct an assessment about someone’s history of homelessness and level of need (currently the Vulnerability Assessment Tool [VAT]). ❖ Prioritization: Prioritize most vulnerable based on assessment and case conferencing.
❖ Referral: Refer to a variety of projects including emergency shelters, Rapid Rehousing, Permanent Supportive Housing, other housing and homelessness projects, and services.
