B.

Caminar's Housing Intervention Solutions Program (HISP) is designed to address the critical need for housing stability among individuals and families in the City of Fairfield who are experiencing or at risk of homelessness. This program will support participants in securing and maintaining stable housing while also ensuring they adhere to lease agreements and access essential services to sustain long-term housing.

The project will be located within Fairfield and will serve vulnerable individuals and families referred through Resource Connect Solano (RCS), the Coordinated Entry System (CES), and Change and New Beginnings ACCESS Outreach Team, which will play a key role in identifying unsheltered individuals and connecting them to the Housing Intervention Solutions Program (HISP) for further support.

To ensure that the most vulnerable individuals in Fairfield are prioritized for housing, the HISP program will collaborate with RCS to establish a dynamic prioritization system. This system will enable individuals who are unhoused in Fairfield to be prioritized and directly referred to the program, ensuring that those with the highest need and are eligible receive timely access to housing support and resources.

By leveraging this dynamic prioritization approach, the HISP Program will enhance its ability to provide targeted assistance, helping to reduce homelessness and foster long-term housing stability for Fairfield's most vulnerable residents.

B. SCOPE OF WORK
T
he program's primary activities include housing search assistance, individualized case management, and tenancy support services aimed at helping clients overcome barriers to securing permanent housing. Once housed, participants will receive ongoing case management, including regular home visits, to ensure lease agreement compliance and address emerging needs. The program will also provide vocational assistance, referrals to behavioral health services, and support for accessing other critical resources, such as transportation and other community services, as needed.

In addition, the Housing Solutions Program will leverage its partnership with RCS and collaborate closely with the RCS Housing Locator to engage landlords throughout Fairfield. Caminar has established strong relationships with local landlords through RCS, providing an existing network of housing options for clients in the program. This collaboration will allow the program to tap into these landlord relationships to secure additional housing placements, ensuring a steady flow of available units for program participants.

The program will maintain a caseload of 30 clients, with a dedicated Case Manager hired to provide intensive support and help clients navigate the housing process, ensuring long-term stability and success.

Data Tracking and Reporting: Using the Homeless Management Information System (HMIS), the HISP program will track enrollments, outcomes, and service provision to ensure a data-driven approach to improving outcomes for Fairfield's homeless residents. The ACCESS team will collaborate with Pathways MISI, the HMIS Lead for the Continuum of Care (CoC), to create a Housing Intervention Solutions Program within HMIS. All individuals will have an HMIS profile created and will be enrolled in the HISP Program, with all services and interactions meticulously documented. In addition to being enrolled in the Street Outreach program prior to being referred to the HISP program, individuals will be dually enrolled Exhibit A

in RCS and assessed using the Housing Needs Assessment (HNA). This will ensure that individuals are placed on the community queue for prioritization and referrals to other housing programs such as Rapid Re-Housing (RRH) and Permanent Supportive Housing (PSH), ensuring a seamless pathway to housing and supportive services.

Sustainability Beyond City Funding Caminar understands that the City of Fairfield will be providing one-time funding; however, Caminar will seek replacement funding for the HISP program through a variety of funders, including local, County, State, Federal, and Corporate Foundation opportunities. For example, if Caminar is awarded the full contract amount, it will have enough funds through the City of Fairfield to operate for 2 years, serving only Fairfield residents. Caminar will apply for Fairfield Tenancy Support Services funds if the funding is available again. Through the RCS program, Caminar has been servicing 41% (346/836) heads of households who are Fairfield residents. Caminar will actively pursue matching and replacement funds, as well as other sources of revenue and unique partnerships, to continue providing Tenancy Services in Fairfield.

The Caminar Housing Intervention Solutions Program will significantly increase the capacity to identify and address the needs of the unhoused population by expanding outreach efforts and enhancing coordination through RCS. This project will work closely with the ACCESS team to identify unsheltered individuals, assess their needs, and connect them to appropriate services, ensuring that individuals disconnected from existing systems receive timely support.

Increasing Capacity to Identify Needs: The program will employ targeted daily outreach in key areas of Fairfield through the ACCESS team, identifying individuals not yet connected to services. By working directly in the field and leveraging Coordinated Entry through RCS, the program will use a comprehensive assessment process to identify housing barriers, such as lack of documentation, mental health challenges, or substance use disorders.

Improving Services for the Unhoused: The case manager will work closely with participants, providing personalized housing stabilization plans that address their specific barriers to securing and maintaining permanent housing. Through HMIS, the program will track client progress, ensuring that all services, including housing navigation and supportive services, are well-coordinated and avoid duplication. The use of harm reduction materials during outreach will further improve service quality, meeting individuals where they are and offering practical tools to enhance their safety and well-being.

Eliminating Duplicate Services: By integrating into Resource Connect Solano's Coordinated Entry System and using HMIS to track client interactions, the program will eliminate duplicate services.

Coordinated care plans will ensure that each participant is linked to the most appropriate services, whether that be housing navigation, mental health support, or substance use treatment. Regular collaboration with other service providers, including behavioral health agencies and local shelters, will further ensure that services are streamlined and effectively targeted.

Increasing the Number of Individuals Assisted: The Caminar Housing Intervention Solutions Program is designed to serve 30 individuals for 15 months. Participants will receive services, including housing navigation and tenancy support, for the duration of the program or until additional permanent housing is found, depending on individual needs. Of the 30 individuals served, the program aims to transition at least 25 participants into permanent housing. By leveraging the established relationships of RCS Housing Housing Intervention Solutions Program

Locator and Caminar's Supportive Housing Team, the program will increase the number of individuals who moved out of homelessness and into stable housing.

Through this structured approach, the program will effectively identify the needs of the unhoused population, provide enhanced and coordinated services, and help more individuals transition out of homelessness into long-term housing solutions.

C. The Caminar Housing Solutions Program will focus on two key measurable outcomes to track the success of the project in supporting individuals experiencing homelessness in the City of Fairfield:

1. Rate of Sheltered: The program will aim to support a total of 30 Fairfield individuals over the duration of the contract period in securing temporary or emergency shelter placements. These placements will provide immediate housing stability for individuals while they work with the case manager to transition into permanent housing options.

2. Rate of Housed: The program will aim to transition at least 25 individuals into permanent housing over the course of the contract period. By working closely with landlords, providing ongoing tenancy support, and connecting participants to needed resources, the program will ensure long-term housing stability for individuals referred through Resource Connect Solano.

These outcomes will be tracked using the Homeless Management Information System (HMIS), ensuring participants' progress toward securing shelter and housing is carefully monitored and evaluated for program effectiveness.

D. The Caminar Housing Solutions Program is designed for an immediate and effective start-up, leveraging Caminar's well-established resources and relationships. The program is set to launch in April 2026, following the contract award in November, and we will capitalize on the infrastructure and partnerships already in place through Caminar's Supportive Housing Team and Resource Connect Solano.

From the outset, we will engage Resource Connect Solano's Housing Locator, who has developed strong, trusted relationships with landlords in Fairfield. These existing connections will allow us to secure additional housing placements for program participants, eliminating delays that are often encountered when identifying housing opportunities for those experiencing homelessness. Additionally, Caminar's Supportive Housing Team has extensive experience in housing navigation and working directly with landlords, giving us an added advantage in placing clients into stable housing as soon as the program launches. The combination of these resources ensures that the program can begin enrolling clients and securing housing immediately in April 2026 , allowing for a seamless transition into tenancy support services.

By leveraging these existing resources and partnerships, the Caminar HISP program is positioned for immediate impact, beginning in April 2026, with a clear plan to secure housing, provide supportive services, and help clients maintain stability through the entirety of the program.
