AMENDMENT NO. 2 TO AFTER SCHOOL EDUCATION AND SAFETY GRANT PROGRAM SERVICES
AGREEMENT
This amendment, dated February 2, 2022, hereby amends the Agreement dated July 1, 2020, between the Fontana Unified School District (DISTRICT), and the City of Fontana (PROVIDER) to provide After School Educational and Safety GRANT (GRANT) Program Services (Program) to DISTRICT pupil s to provide additional enrichment programs as established by the California Department of Education (CDE), inclusive of the Expanded Learning Opportunities Program (ELO-P). The following terms are changed to:
• Change of Agreement to include all Expanded Learning Program Services related to the GRANT . • GRANT award in the estimated amount of $152,612.13 per e lementary school and $203,482.84 per middle school based on the Average Daily Attendance calculation of $10.18/day per student. • Additional funding in the estimated amount of $63,957,520 to provide Expanded Learning Program Services as related to the GRANT and established by CDE. • Access to the Program to increase to 50% of currently enrolled unduplicated pupils in school year 2021/2022 (Schedule “A”) and 100% beginning school year 2022/2023 (Schedule “B”).
