Symba Center is a 501 (c)(3) non-profit (84-3729902) community-based organization and medical clinic providing health and wellness services for unhoused, low-income, and uninsured community members in the High Desert and greater Inland Empire. Our mission is to implement and inspire innovative solutions designed to meet the health and wellness needs of our community. Our objective is to minimize plight and suffering related to health conditions for homeless, low-income, and uninsured community members. Currently, we are the Wraparound Services Provider of the City of Victorville’s Wellness Center that can provide recuperative care and short-term hospitalization for individuals in need of such services. The innovative design of the Wellness Center allows us to provide services to 170 individuals in a non-congregate environment that allows for case management, behavioral health, and medical services conducive to moving the client forward in their wellness in a holistic manner. The design of a one stop shop for wellness services allows for clients to heal and attend to themselves with dignity and respect. Symba Center is one of the first recuperative care providers in the County of San Bernardino. Our multi-disciplinary team of Case Managers/Housing Navigators, Peer Support Specialists, Nurse Practitioners, Registered Nurses, Medical Assistants, Occupational Therapists, Pharmacists, Licensed Vocational Nurses, and Medical Assistants support all individuals on their road to recovery and wellness and helping the individual remain focus on the primary objective of finding a permanent housing solution. Our use of evidenced based modalities, Housing First Principles and Motivational Interviewing techniques assure that we remain client- centered and are moving clients through a wellness continuum designed to support securing housing and developing a network of natural supports within their community that is supportive to reducing recidivism. Our goal is to provide the City of Fontana Motel with four (4) Case managers/Housing Navigators, one (1) Peer Support Specialist a (1) Director whose primary goal as a team will be to identify barriers to housing and help develop wellness plans to overcome barriers and challenges to housing. We will also have vehicles available to transport clients to critical appointments. Please see the budget below. Our Wellness Team will provide one-on-one support and help make connections to transitional housing programs, health services, public benefits, and permanent housing solutions within the community supportive to client’s overall well-being. These services will involve collaboration with other organizations and government agencies such as the Department of Behavioral Health. The creation of Housing/Wellness Plans with the client provides crucial support to individuals experiencing homelessness by helping them to navigate the housing market and connecting them to necessary services that will ensure the individual has all the available tools to a successful transition from the shelter and into permanent housing. Through our contracts with IEHP including Housing Tenancy and Sustaining Services and Housing Deposits to help ensure housing stability, and collaboration with IEHP Housing Team as needed to address any issues that may jeopardize housing once a client has "graduated" from Housing Transition

Navigation services. Through IEHP, we will be able to provide upfront housing costs for clients which will reduce barriers to entry into affordable housing Lastly, this partnering directly speaks to the overall mission of Symba Center mission of bringing accessible services for the homeless and at-risk community throughout San Bernardino County. This joint effort will eventually expand to bring health care services, substance use counseling programs, chronic condition management, and interventions for the homeless and the community of Fontana and make a positive impact on reducing recidivism.
