You are hereby notified that your proposal has been accepted. You are the successful bidder and will be awarded a Purchase Order contract.
In accordance with the contract Terms, you must comply with and complete the following conditions of this Notice of Award within 10 days.
1 . Worker’s compensation insurance
2. Commercial general liability insurance
3. Commercial auto liability and property insurance.
4. Professional liability insurance.
5. Name, title, and email address of account representative for this account.
6. Name, title, and email address of personnel authorized to sign/approve this contract.
7. Email address that purchase orders will be emailed to.
8. Completed ACH form – if wanted to be paid electronically.
9. Taxpayer ID and certification W-9.
10. Copy of City of Fontana business License.

ote: Insurance certificates must name City of Fontana as additional insured and be in the amounts specified in the bid specifications. With all endorsement forms.

ailure to comply with these conditions within the time specified will result in annulment of the award and forfeiture of the bid guarantee. Within 10 days after you comply with the foregoing conditions, the City of Fontana will issue you a purchase order.
