Based on mutual agreement, Apex Testing Laboratories, Inc. and City of Foster City agree to amend the original Agreement dated October 11, 2024 (“Agreement”) pursuant to Section 17 of the Agreement to increase the compensation amount identified in Section 3 and add additional services relating to the Project for specified amounts, to Exhibit A (Scope of Services) identified in the Agreement as follows:

AMOUNT OF CONTRACT AMENDMENT NO. ONE (1) $ 30,000
AMOUNT OF CONTRACT AMENDMENT NO. TWO (2) $ 35,000
REVISED CONTRACT AMOUNT $ 406,520
Additional work included in the contract amendment:
Task #1: Additional Testing and Inspection Services
1. Continue assisting with testing and inspection services for the Community Center Rebuild Project CIP678, which includes all tasks listed in the original contract and additional testing and inspection such as waterproof testing.

Except as specifically modified herein, all other terms and provisions in the Agreement shall remain unchanged.

Apex Testing Laboratories, Inc. have given careful consideration to the work proposed and hereby agree, if this proposal is approved, to provide all equipment, furnish all materials except as may otherwise be noted above, and to perform all services necessary for the work above specified, and to accept as full payment therefore the prices shown above.
