Executive Summary: Since 2001, the State of California’s Multipurpose Senior Services Program (MSSP) has contracted with the City’s Human Services Department to provide intensive case management services to 49 older adults (65+) who are nursing home eligible but prefer to live at home.
For the first time in 20 years, the State has expanded the number of MSSP “slots” in Alameda County and has allocated an additional 53 slots to the City of Fremont, for a new total of 102 clients, at a rate of $5,356/year.
Staff recommends the City Council authorize the City Manager to execute an amendment with the California Department of Aging (CDA), for MSSP services, through FY 2021/22 in the amount of $404,348.
BACKGROUND: Since 2001, CDA has contracted with the City to provide MSSP case management services, through the Human Services Department’s Aging and Family Services Division, to low-income, frail older adults in Southern Alameda County. This program has historically provided services to forty-nine very frail individuals at any one time. The care management team is comprised of a social worker and nurse who develop and monitor a plan of care that includes regular home visits, medical care coordination, medication management, health education, and the purchase of necessary mobility and home safety devices.
The CDA has offered the City an amendment to its FY 2021/22 contract that increases the number of individuals that can be served at any one time to 102. The 53 additional “slots” for services are available January 2022 at the current rate of $5,356 per client per year for a prorated total of $141,929 through June 30, 2022.
DISCUSSION/ANALYSIS: All of the City’s MSSP clients will be certified by a nurse as eligible for nursing home level of care. The average age of clients is 80 years and almost half of them live alone. MSSP clients take an average of 10 medications and require substantial assistance in coordinating their medical care and social needs. With the support of this program, clients can attain their goal of living at home.
Staff Report (ID # 4507) Meeting of January 11, 2022
Updated: 1/4/2022 9:29 AM by Susan Gauthier Page 2
The Tri-City area is experiencing a demographic shift in which the older adult population is growing at a faster rate than the total population. As an increasing number of older adult residents require support to remain safely living in the community, programs such as MSSP are going to be increasingly in demand.
FISCAL IMPACT:
MSSP Revenue: The MSSP contract reimburses the City at a rate of $5,356 per client per year. The current contract for 49 client slots is for $262,454. The proposed $141,929 contract amendment will support the additional 53 slots for 6 months, thus increasing the FY 2021/22 contract to $404,383. Starting fiscal year 2022/23, the contract will increase to $546,333.

per year Months Slots Total
Current Agreement $5,356 12 49 $262,454
6-month Amendment $5,356 6 53 $141,929
Amended Agreement 102 $404,383
Program Costs: In order to meet MSSP’s required ratio of clients to care manager, the additional MSSP revenue will be used to increase case manager hours from 1.75 FTE to 2.80 FTE. Staff is also proposing to increase billing and support staff hours from .35 FTE to .50 FTE. The contract will also cover the purchase of equipment and services for the additional clients, as well as internal service and administrative overhead costs.
ENVIRONMENTAL REVIEW: The action is exempt from CEQA pursuant to, without limitation, CEQA Guidelines Section 15378(b)(2), (4) and/or (5) and/or 15061(b)(3), each as a separate and independent basis for CEQA exemption, as it is a fiscal or administrative activity of a government agency that has no possibility, either directly or indirectly, to cause a significant effect on the environment.
