Executive Summary: The Fremont Fire Department (FFD) equips each engine and truck with a mechanical chest compression device to provide continuous CPR during transport to the hospital. These devices have a seven-year life and are now due for replacement, which is budgeted through the Fire Capital Replacement Fund. The purpose of this report is to authorize the City Manager or designee to issue a purchase order to Stryker Corporation for $245,119 to purchase 16 LUCAS Chest Compression Systems.
Because one of the LUCAS systems will be used by the new fire truck company once it becomes operational, staff is also recommending that the City Council appropriate the cost of that system in the Fire Impact Fee Fund.
BACKGROUND: In 2012, the Alameda County EMS Authority mandated that all county fire departments’ first responder paramedic engines and trucks be equipped with a mechanical chest compression device, thus leading to mechanical CPR as the standard while providing care on the scene of a cardiac arrest.
In the last 18 months (September 2021 to March 2023) FFD responded to 476 cardiac arrests. Although not all of the 476 responses resulted in the need for CPR, this number still emphasizes this equipment's importance. CPR offers the best survival rate for a cardiac arrest. Fremont Fire Department currently uses the Zoll AutoPulse for automated CPRand are due for replacement.
DISCUSSION/ANALYSIS: A field study was conducted to determine the best device for Fremont Fire’s first responders and our community. The Stryker LUCAS Chest Compression System was ranked the highest. In addition, Alameda County EMS, ambulance contractors, Alameda County Fire and other agencies in the county are now using the LUCAS, making it a standard within the county.
The LUCAS Device has been adopted for use in Alameda County Hospital Emergency Departments as the preferred method of mechanical CPR. Many other Fire Agencies in Alameda County utilize the LUCAS device making it preferable in the event of a Mutual Aid Event.
Council Staff Report (ID # 4839) Meeting of April 4, 2023 Updated: 3/28/2023 2:45 PM by Susan Gauthier A Page 2
The LUCAS Devices will be placed in thirteen front line apparatus, one will be used for the 14th Company (once operational) and the remaining two will be used as spares.
This purchase has been approved as exempt from the competitive procurement requirements pursuant to Fremont Municipal Code, Chapter 3.20, Article 7. Staff consulted with the Purchasing Division, and based on the discounted purchase price the City is receiving, and the high-quality, compatibility and broad-based use of the Lucas Chest Compression Systems, bidding will not be useful or produce any advantages to the City.
FISCAL IMPACT: The Fire Capital Replacement Fund (Fund 615) adopted budget has sufficient available appropriations to purchase 15 of the recommended LUCAS systems.
Staff is recommending that the City Council appropriate $15,320 in the Fire Impact Fee Fund (Fund 544) to pay for the 16th system, which will be used to equip the new fire truck company when it becomes operational.
ENVIRONMENTAL REVIEW: The item is not subject to review under the California Environmental Quality Act (CEQA) per CEQA Guidelines sections 15378 because the action is an administrative governmental activity that will not result in direct or indirect physical changes in the environment.
