Executive Summary: The Human Service Department has a long-standing partnership with the Alameda County Social Services Agency (ACSSA) to provide services to older adults residing in the Tri-City area. ACSSA proposes to renew this partnership with a three-year contract for four services to be provided by Aging and Family Services Division staff.

Staff recommends the City Council authorize the City Manager to execute four contracts with the ACSSA totaling $1,393,500 for the period of three fiscal years: July 1, 2023 through June 30, 2026, including contracts not to exceed:

1) $136,000 to provide Age Well Center activities,
2) $473,000 to provide Family Caregiver Support,
3) $460,000 to provide Information and Assistance, and
4) $324,000 to provide senior Case Management.

BACKGROUND: ACSSA has a long history of contracting with the Human Services Department (HSD) to provide services to older adults living in the Tri-City Area.

Historically, the contracts have been established on an annual basis. This year, ACSSA is proposing three-year contracts that include FY 2023/24, FY 2024/25 and FY 2025/26.

The contracts are specifically to provide 1) Senior Center Activities through the City’s Age Well Centers; 2) Family Caregiver Support Services to family members who are caring for an older adult; 3) Case Management service to develop individualized assessments and care plans to help guide older adults; and 4) Information and Assistance to provide linkages to programs that support older adults.

DISCUSSION/ANALYSIS: The three cities that comprise the Tri-City area of Southern Alameda County (Fremont, Union City and Newark) have 51,000 residents over the age of 60 years. In Fremont, the largest of the three cities, a full 18% of the population is over the age of 60 years, a percentage that is growing.

HSD has provided services to older adults since 1978. Most clients are older adults who are living with chronic conditions and functional limitations. ACSSA funding supports the following programs:

Council Staff Report (ID # 4989) Meeting of November 7, 2023 Updated: 10/31/2023 9:18 AM by Susan Gauthier Page 2
Age Well Center Activities: This program enables adults to attain and maintain physical and mental well-being, as well as civic engagement, through the provision of services, classes and volunteer opportunities available at the City’s two Age Well Centers.

Family Caregiver Program: The City of Fremont Family Caregiver Program provides a professional assessment of caregiver needs, individual counseling for caregivers, support groups, and educational workshops, all of which are designed to specifically support family caregivers and reduce caregiver stress. The program provides individual assessment and counseling to 110 caregivers and care receivers each year. Caregiver workshops, which are open to the community, occur several times a year. Dedicated caregiver support groups are offered on a weekly basis at different times and locations, including on-line.

Case Management: This program supports a collaborative process between City staff and community members that assesses, plans, implements, coordinates, and monitors services to meet an individual’s health, financial, and social needs. The program’s goal is to enable individuals 60 years and over to obtain services that promote and maintain their optimal functioning in the least restrictive setting possible. Clients have complex medical conditions, functional limitations, and social challenges.

Information and Assistance: The goal of the Information and Assistance program is to link older individuals who need assistance to the appropriate services.

FISCAL IMPACT: In anticipation of these programs, the City Council appropriated $275,000 in the FY 2023/24 Adopted Operating Budget. The actual total combined FY 2023/24 contract amount is $504,593; therefore, staff is recommending the City Council appropriate an additional $229,593, including $198,911 in Area Agency on Ageing Fund 173 and $30,682 in Adult and Family Services Fund 106. Contract funds offset the cost of staff that provide the contracted services. Collectively, over the three-year period (FY 2023/24, FY 2024/25, and FY 2025/26), the contracts will not exceed $1,393,500.
