Executive Summary: The purpose of this report is to recommend that the City Council adopt a resolution authorizing the City Manager, or designee, to execute an agreement for Fiscal Year 2025/26 with the California Department of Aging (CDA) in the amount of $546,312 for the City to administer the Multipurpose Senior Services Program (MSSP), which provides case management and purchased services for frail, Medi-Cal eligible, homebound seniors in Southern Alameda County.
BACKGROUND: Since 2001, CDA has contracted with the City to provide MSSP case management services in Southern Alameda County through the Human Services Department. The primary purposes of the MSSP are (1) to keep low-income, nursing home eligible seniors out of nursing facilities and in their homes for as long as possible and (2) to foster independent living in local communities. The City will provide services for up to 102 MSSP eligible individuals at any one time in Southern Alameda County (Fremont, Hayward, Newark, and Union City). The care management team is comprised of three case managers, specializing in social work and nursing, who develop and monitor a plan-of-care that includes regular home visits, medical care coordination, medication management, health education, and the purchase of necessary mobility and home safety devices.
DISCUSSION/ANALYSIS: The age of the City’s MSSP clients ranges from 69 to 104 years.
Eighty percent of current clients are female and almost half of them live alone. The clientele is diverse with 30% being Caucasian, 18% Chinese (primarily mono-lingual), 15% Afghan, 9% African American and 28% other. MSSP clients take an average of 11 medications and require substantial assistance in coordinating their medical care and social needs. All MSSP clients will be certified by a nurse as eligible for nursing home level of care. However, older adults generally wish to remain at home, and with the support of the MSSP, clients can attain their goal of living at home as long as possible.
The Tri-City area is experiencing a demographic shift in which the older adult population is growing at a faster rate than the total population. As an increasing number of residents require support to remain living safely in the community, programs such as MSSP play an important role in addressing the needs of this population.

FISCAL IMPACT: CDA will reimburse the City at a per client per month rate, for a total contract amount not to exceed $546,312 for FY 2025/26. The funding received under the proposed agreement with CDA will support the staffing, supplies, services and administrative costs of the MSSP. Currently, the City can serve up to 102 clients at any point in time. Revenues and costs associated with the MSSP will be recognized and appropriated as part of the FY 2025/26 Proposed Operating Budget.
ENVIRONMENTAL REVIEW: The City Council’s action on this resolution is exempt from the California Environmental Quality Act pursuant to, without limitation, CEQA Guidelines Section 15378(b)(2), (4)-(5), as an organizational, administrative, policy, and fiscal activity that will not result in direct or indirect physical changes in the environment, and Section 15061(b)(3), as it can be seen with certainty that there will be no possibility that the activity may have a significant effect on the environment, each as a separate and independent basis.
