Executive Summary: As part of Medi-Cal’s system transformation, known as California Advancing and Innovating Medi-Cal (CalAIM), the California Department of Health Care Services (DHCS) has awarded the City of Fremont (City) a grant through the Capacity and Infrastructure Transition, Expansion and Development Program - Intergovernmental Transfer (CITED-IGT), which is part of the California Providing Access and Transforming Health (PATH) Initiative. To receive the grant award, the City must provide a 50% local funding match. The City has been awarded a CITED-IGT Grant of $211,911.11 and the City must provide matching funding in the amount of $211,911.11, for a total agreement amount of $423,822.22. This Grant award will allow the Human Services Department (Human Services) to provide additional Housing Community Supports to unhoused older adults.

Staff recommends that the City Council authorize the City Manager, or designee, to execute an agreement with DHCS to accept the CITED-IGT Grant in an amount not to exceed $423,822.22, which includes the required $211,911.11 in matching funds from the City.

BACKGROUND: Medi-Cal, California’s Medicaid health care program, pays for services for children and adults with limited income and resources. The Medi-Cal program is undergoing a transformative system change to CalAIM, with the goal of:

• Improving population health using a whole person care model and social determinants of health;
• Reducing complexity and variation, and increasing flexibility of services across Medi-Cal; and
• Improving quality outcomes, reducing health disparities, and modernizing systems through value-based initiatives and payment reform.

As part of this CalAIM transition, in 2023, Alameda County Health’s (ACH) Office of Housing and Homelessness Services spearheaded the County’s effort to expand the Housing Community Supports (HCS) program for unhoused populations. HCS includes housing transition navigation services, tenancy and sustaining services and one-time flexible housing deposits. The program is funded through a combination of CalAIM funding from the Alameda Alliance for Health, Kaiser Permanente, and additional Alameda County general funds.

ACH contracts with Human Services to carry a caseload of up to 65 unsheltered eligible clients age 18+ and Human Services currently carries a caseload of approximately 40 clients, utilizing Case Managers from the Fremont Family Resource Center. HCS has been a self-sustaining program for Human Services, with staff and supervision costs covered by Medi-Cal reimbursements.

In November 2025, DHCS selected the City to receive a CITED-IGT Round 4 Grant award.

DHCS is offering the City a total CITED-IGT Grant award of $423,982.22, which includes the 50% federal funding award from the CITED-IGT program and the City’s required 50% funding match.

DISCUSSION/ANALYSIS: The CITED-IGT Grant funding opportunity is intended to support infrastructure, expansion, and development of Housing Community Supports services. This funding will allow the Human Services’ division of Aging and Family Services to fill a vacant case manager position that will provide HCS to 25 unsheltered seniors under the current agreement with the ACH. It will ensure financial sustainability of the HCS program as the new Case Manager builds up to a full caseload of 25 clients.

The CITED-IGT funding priorities align directly with the goals of Human Services’ Aging and Family Services Older Adult Support Program and the City’s Homelessness Response Plan.

Funding would cover expenses related to maintaining program quality.

FISCAL IMPACT: There will be no impact to the City’s General Fund. Human Services intends to utilize $211,911.11 in existing unallocated local match fund balance in the Human Services Special Revenue Fund (Fund 102) to draw down the $211,911.11 in required local match funding, for a total award of $423,822.22.

ENVIRONMENTAL REVIEW: The proposed action is exempt from the California Environmental Quality Act (CEQA) because, without limitation, it is not a “project” as defined under CEQA Guidelines Sections 15378(b)(2), (4), (5), and it can be seen with certainty that the action has no possibility of causing a significant effect on the environment under CEQA Guidelines Section 15061(b)(3), each as a separate and independent basis for CEQA exemption.

ATTACHMENTS: None.

RECOMMENDATIONS: Authorize the City Manager, or designee, to execute an agreement with DHCS to receive the CITED-IGT Round 4 Grant award in an amount not to exceed $423,822.22, which includes the required 50% City match funding contribution of 211,991.11.
