RESOLUTION NO. 2026-XX
A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF FREMONT
AUTHORIZING THE CITY MANAGER OR DESIGNEE TO EXECUTE
AGREEMENT MS -2627-37 WITH THE CALIFORNIA DEPARTMENT
OF AGING FOR THE CITY TO PROVIDE SERVICES UNDER THE
MULTI-PURPOSE SENIOR SERVICES PROGRAM FOR FY 26-27
WHEREAS, the Multipurpose Senior Services Program (MSSP) is a program run by the State of California’s Department of Aging (DCA) that provides funding to enable frail, Medi-Cal eligible seniors aged 60 and over to remain safely at home rather than enter nursing homes; and
WHEREAS, since February 2001, the City of Fremont (City) has contract ed with the DCA to administer and provide MSSP ca se management services in Southern Alameda County through the City’s Human Services Department; and
WHEREAS, the City desires to continue its successful collaboration with DCA on the MSSP to continue providing services to help keep low-income, frail, nursing home eligible seniors out of nursing facilities and in their homes for as long as possible .
NOW, THEREFORE, THE CITY COUNCIL OF THE CITY OF FREMONT RESOLVES that the City Manager, or designee, is hereby authorized to execute Agreement MS- 2627-37 with the California Department of Aging for an amount not-to-exceed amount of $546,312 for the City to administer and provide MSSP services for Fiscal Year 2026-2027 and to take all other actions necessary to implement Agreement MS-2627-37.
BE IT FURTHER RESOLVED that a copy of this resolution be transmitted to the State of California Department of Aging .
