Executive Summary: The City’s Human Services (HS) Department partners with the Alameda County Social Services Agency (ACSSA) to deliver critical services to older adults in the Tri-City area. In 2026, the Alameda County Board of Supervisors approved extending and increasing funding through December 2026 for two existing City agreements —Family Caregiver Support and Case Management. The older adult services provided by the City under these agreements align with the domains of Health and Wellness, Housing, and Dementia-Related Support outlined in the Age-Friendly Fremont Action Plan Update 2025.

To accept this additional funding, City staff recommends that the City Council authorize the City Manager, or designee, to execute amendments to the City’s two current agreements with the ACSSA, to revise the FY 2026/27 budgets and increase the not-to-exceed amounts as follows:
• For Family Caregiver Support, from $569,235 to $664,108. • For Case Management, from $332,046 to 451,914.

Staff also recommend that the City Council increase the revenue estimates and appropriate additional funding in the Area Agency on Aging Fund and the Adult and Family Services Fund to reflect the increase in funding being received from ACSSA.

BACKGROUND: ACSSA has historically contracted with the City to provide key services to older adults, including:

1. Family Caregiver Support Services – assisting individuals by providing care to aging adults.

Council Staff Report (ID # 5768) Meeting of July 14, 2026 Updated: 7/10/2026 9:27 AM by Alberto Quintanilla A Page 2

2. Case Management Services – assessments, care planning, and ongoing support for older adults needing help maintaining stability and independence.

The City is currently in the final year of three-year agreements with ACSSA for FY 2023/24–FY 2025/26. Because of delays in ACSSA’s countywide procurement cycle, ACSSA elected to extend funding for two of the four City-held social service contracts through December 2026, with updated FY 2026/27 budgets.

DISCUSSION/ANALYSIS: The City is currently in the final year of three- year agreements with ACSSA covering FY 2023/24, FY 2024/25, and FY 2025/26. To seek funding to continue providing these services to older adults, the HS Department has completed proposal packets to receive funding from ACSSA for the next contract cycle (through FY 2029/30).

Services provided under the City’s agreements with ACSSA align with the domains of Health and Wellness, Housing, and Dementia-Related Support found in the Age-Friendly Fremont Action Plan Update 2025.

Due to delays in the award process, ACSSA is amending FY 2025/26 contracts for two of the four agreements with the City for older adult services to extend services through December 2026 and to revise the FY 2026/27 budget and increase the not-to-exceed amounts under the agreements by the following amounts:
• For Family Caregiver Support, from $569,235 to $664,108. • For Case Management, from $332,046 to $451,914.

Staff recommend the City Council authorize the City Manager to amend the two current agreements with ACSSA to accept additional funding to provide continuing services to older adults.

FISCAL IMPACT: In anticipation of providing older adult services under the current agreements with ACSSA, the City Council appropriated amounts for these programs in the FY 2026/27 Adopted Operating Budget. The compensation received under these ACSSA agreements offsets the costs associated with HSD staff providing the contracted services.

The FY 2026/27 appropriation for the Family Caregiver Support Program in the Area Agency on Aging Fund (Fund 173) is sufficient to accommodate the amended agreement. In addition, Staff recommend that the City Council increase the revenue estimate and expenditure appropriations by $9,186 for Council Staff Report (ID # 5768) Meeting of July 14, 2026 Updated: 7/10/2026 9:27 AM by Alberto Quintanilla A Page 3 Case Management Services in the AFS City Partners Fund (Fund 106) to reflect the increase in funding for FY 2026/27 under the amended agreements with ACSSA.

ENVIRONMENTAL REVIEW: The proposed amendments to existing agreements are exempt from and not subject to review under the California Environmental Quality Act (CEQA) because, without limitation, neither are a “project” under CEQA Guidelines Section 15378(b)(2), 15378(b)(5), and/or 15378(b)(5), and/or the actions have no possibility of having a significant effect on the environment, as specified in CEQA Guidelines Section 15061(b)(3), each as a separate and independent basis for CEQA compliance.

ATTACHMENTS: None.

RECOMMENDATIONS:

1. Authorize the City Manager, or designee, to amend two agreements with the Alameda County Social Services Agency to extend the terms through December 31, 2026, revise the FY 2026/27 budgets for older adult services, and increase the not-to-exceed amounts for each agreement as follows:
a. For Family Caregiver Support, from $569,235 to 664,108. b. For Case Management, from $332,046 to $451,914.

2. Increase the revenue estimate and appropriate an additional $9,186 to the AFS City Partners, AFS OASS Case Management (Fund 106).
