Artistic Crosswalk Location
Indicate the location of the proposed artistic crosswalk (write a description below and attach site map).
Please note that in order to be considered, the location must:
• Have an existing marked crosswalk or meet the City’s standards for the installation of a marked crosswalk.
• Be at a location where vehicles are currently required to stop by stop sign or traffic signal. • Be at a location where the speed limit is not exceeding 30 MPH and has a maximum of two through lanes.
• Shall not extend to sidewalks, curbs, or gutters.
• Have an asphalt surface.

For the following section, please note:
• The crosswalk shall contain two white (yellow in school zones) transverse (horizontal) lines. • The design shall not impair the visibility of the transverse crosswalk lines. • The design shall be solely for aesthetic purposes (for more information on this, see Artistic Crosswalk Policy, 220.02).

Additional Permit Requirements
For the following section, please note the following:
• A special event permit must be obtained prior to the consideration of this artistic crosswalk application.
• As a condition of the special event permit, all insurance information must be up-to-date and valid for the installation of the artistic crosswalk.
