Project I.D. FA00007

Contract Change Order No. 01 provides for additional full depth concrete repairs and additional contract working days.

Item A: During Construction, RFI No. 01 and 02 were provided by the contractor requesting direction on how to proceed with two (2) locations which had significantly changed in condition since planning. Both locations on the yellow level along gridline 6.5 between lines D -E and along gridline 10.8 between lines D -E were identified as crack injections on the approved plans. The locations now need additional repairs in order to adequately resolve the current conditions. The City’s consultant provided direction to implement full depth concrete restoration, including subgrade compaction, vapor barrier, reinforcement, and concrete placement. There will be an additional twelve (12) working days added to the Contract Time for this item.

Item B: Provides for eight (8) working days to be added to the Contract Time to perform the subgrade compaction requirements as identified in the response to RFI No. 10. All costs associated with this scope will be paid under Contract Bid Item No. 17 – Supplemental Work.

Funds are available in the Project Account to cover the cost of this Contract Change Order.

Staff has reviewed this work and recommends approval of this Change Order.

There will be eighteen (18) Working Days added in the C ontract Time by this Change Order.
