AMENDMENT NUMBER ONE TO BROKER SERVICES AGREEMENT BETWEEN
THE CITY OF FULLERTON AND BURNHAM BENEFITS INSURANCE SERVICES THIS AMENDMENT amends the Broker Services Agreement (Agreement) dated January 1, 2019 between the City of Fullerton and (City) and Burnham Benefits Insurance Services (Burnham).
FOR GOOD AND VALUABLE CONSIDERATION, the receipt and sufficiency of which are hereby acknowledged, the undersigned parties agree as follows:
1. Section 2-Term of the Agreement. City and Burnham exercise the option to amend the Agreement through December 31, 2023.
2. Section 5-Compensation of the Agreement. Broker shall be compensated through direct payment of broker fees at the following annual rates:
January 1, 2022 - December 31, 2022 $75,000
January 1, 2023 - December 31, 2023 [$77,250
3. Continuation of Agreement - Except as amended herein, the terms and conditions of the Agreement shall remain in full force and effect.
4. Binding Agreement - City and Burnham hereto acknowledge that each has read this Amendment, understand it, and agree to be bound by its terms and conditions as stated herein.
