Service Coordinator Designation Form
Section 3.5(C) of the WISE Agreement reads as follows:
“Service Coordinator. The County and the City each shall designate in writing thirty (30) days prior to the expected Commencement Date a person to transmit instructions, receive information and otherwise coordinate service matters arising pursuant to this Agreement (each a “Service Coordinator”).
