Please prepare to provide the following:
1. Required Certificate of Insurance and all endorsements with the amount accordance to that of stated in the agreement, along with a waiver of subrogation for each of the following policy:
a. Workers’ Compensation
b. Employers’ Liability
c. Commercial General Liability
d. Automobile Liability
