Approved for December 17, 2024 calendar

EXECUTIVE SUMMARY
Supportive Services Program (SSP) provides senior case management services under the Community Services & Parks (CSP) Department. The City receives grant funds from Los Angeles County Aging and Disabilities Department (AD) to provide SSP. Los Angeles County AD has authorized an increase to the Service Delivery Unit Rate under the grant for SSP. The current unit rate of $50.00 per hour will increase to $60 per hour, which will allow the City to be reimbursed at a higher rate for the services provided.

Los Angeles County AD SSP grant funding provides annual assistance for up to 80 unduplicated seniors 60 years and older which include: initial intake, in-home assessment, care plan, information and referral services, purchase of goods/services, annual reassessments, and well-being checkups.

RECOMMENDATION
That the City Council accept and authorize the City Manager to execute Amendment Five to the SSP232404 subaward period July 2024-June 2025 to implement the unit rate increase from Los Angeles County AD for the FY 2024-2025 Supportive Services Program.

ANALYSIS
CSP has been providing the Supportive Services Program (SSP), better known as senior case management since 1992. SSP provides comprehensive, long-term services to seniors 60 years and older, that utilize a case manager’s professional skills and competence to serve senior clients and link them to a full range of appropriate services including service coordination, care planning, purchase of goods/services, benefits enrollment, and referral services. The primary goal is to assist the elderly in maintaining their independence and living in their own home. The SSP is operated through the Senior Services Unit which also provides long term services that include the Elderly Nutrition Program, Emergency Meals, and Telephone Reassurance Program.

CSP was originally awarded an SSP subaward from Los Angeles County AD through a noncompetitive process where the City entered into a subaward agreement for a one- year term, commencing July 1, 2024, through June 30, 2025, with three additional options to renew annually for an anticipated total subaward term of four years. Baseline funding for the program requires a 15% cash match. In addition to baseline funding, SSP also received Modernizing Older Americans Act (MOCA) funding in FY 2024-25 which does not require a cash match.

SSP allows senior case managers to provide program services in the daily operations of the program including home visits, case planning, monitoring, contract compliance, invoicing, purchase of goods/service, and data tracking and direct assistance to seniors for case management services. Examples of direct assistance include payment of a utility bill, purchase of small appliances, home cleaning, clothing, respite, medical equipment/supplies, etc. Time that is spent on SSP services is booked monthly by the case managers as service delivery time per client and is supported by progress notes and other supporting documentation.

SSP assists up to 80 unduplicated seniors sixty years of age and older each fiscal year, with the following senior case management services: initial intake screening, in-home assessment, service authorization, care plan, and connecting to other services. Service areas for SSP include Glendale, La Crescenta, Montrose, and Verdugo City.

On December 3, 2024, LA County AD provided a letter informing CSP that the SSP would receive a unit rate increase from $50.00 per hour to $60.00 per hour. The unit rate increase is effective from contract execution through June 30, 2025 which includes baseline and MOCA funding.

STAKEHOLDERS/OUTREACH
SSP is part of the Senior Services Unit which operates out of the Adult Recreation Center.

Program services are advertised quarterly in the CSP Leisure Guide, CSP Newsletter, on the department website and social media, and outreach is conducted with other community agencies to ensure the public is aware of the available services. 3{{section.number}}h

FISCAL IMPACT
The SSP program receives a total of $139,200 in grant funding which is reimbursable through service delivery. Currently SSP unit rate for service delivery is $50.00 per hour, but will increase to $60 per hour with this amendment.

This unit rate increase does not pose a fiscal impact, as the total grant funding is not changing.

ENVIRONMENTAL REVIEW (CEQA/NEPA)
Because no possibility exists that the activity in question (accepting grant funding) may have a significant effect on the environment, the activity is not subject to CEQA review under the commonsense exemption set forth in 14 Cal. Code Regs § 15061(b)(3).

CAMPAIGN DISCLOSURE
This item is exempt from campaign disclosure requirements.

ALTERNATIVES TO STAFF RECOMMENDATION
Alternative 1: Do not approve unit rate increase from Los Angeles County AD for FY 2024- 25 SSP, which means the City will be reimbursed for its services under a lower rate.

Alternative 2: The City Council may consider any alternative not proposed by staff.
