FOODWARE EXEMPTION REQUEST FORM
Use this form to apply for an exemption from one or more elements of the City of Glendale’s Plastic Waste Reduction Ordinance (Title 8, Chapter 8.42). The City of Glendale adopted the ordinance on DATE to address the growing problem of plastics in our environment created by our disposable culture. The requirements apply to all food vendors such as sit-down restaurants, fast- food restaurants, bars, and hospitality venues. Each exemption request shall be reviewed on a case-by-case basis, and may be granted in whole or in part, with or without conditions, for a period of up to twelve (12) months.
