EXECUTIVE SUMMARY
The City of Glendale received $441,114 in Housing and Homeless Incentive Program (HHIP) funds from L.A. Care and Health Net, LLC to support homelessness prevention, housing retention, and healthcare services for Medi-Cal recipients. A total of $391,114 was allocated to Ascencia, a local non-profit agency, with prior Council approval, to provide intensive case management to 45 chronically homeless households in the City’s Shelter Plus Care (SPC) Rental Assistance Program. The remaining $50,000 was kept in-house to cover staff salaries for Homeless Management Information System (HMIS).
While the funding for this grant began in January of 2023, certain positions funded by HHIP were not filled until August 2023, resulting in unspent funds totaling $284,439.83, with a contract expiration date of December 31, 2024. As a result, Ascencia has requested a no-cost extension to the existing contract, along with a budget modification in order to fully utilize the HHIP funding by August 31, 2025, which with Health Net, LLC and L.A.
Care are agreeable.

RECOMMENDATION
That the City Council pass the Motions to extend the existing Agreements between the City of Glendale and Health Net, LLC, City of Glendale and L.A. Care, and City of Glendale and Ascencia to August 31, 2025.
ANALYSIS
The City of Glendale received $441,114 in Housing and Homelessness Incentive (HHIP) Program from Health Net, LLC and L.A. Care to provide supportive services, housing and healthcare services to those experiencing homelessness or at the risk for a 24-month period, starting January 1, 2023. These funds are being used to assist individuals currently experiencing homelessness or at-risk in the City’s Shelter Plus Care (SPC) Rental Assistance Program, funded by the Department of Housing and Urban Development (HUD).
SPC follows the “Supportive Housing” model, which integrates housing and supportive services. The rental assistance is funded by the HUD Continuum of Care Program (CoC) and provides rental subsidy for 45 units (total 59 chronically homeless households) in the City. SPC prioritizes placement for chronically homeless individuals when new vacancies occur or if there are turnover units. The project participants receive supportive services to promote self-sufficiency and stabilize their housing tenure.
With prior approval from City Council on June 13, 2023, the City entered into a sub- recipient Agreement with Ascencia to continue providing intensive case management services for participants in the SPC Program, such as assistance with housing funding and building relationships with landlords, chronic disease management, and help with addressing mental health or substance abuse treatment. The Program is experiencing an aging population, which requires ongoing case management and community support.
With the use of these funds, Ascencia is required to develop individual case plans to meet the needs of each client and determine housing stability. The HHIP funds also serve as matching funds for the SPC Program, as required by HUD.
Although the total of $391,114 funding was available as of January 2023, Ascencia did not fill critical positions until August 2023, resulting in low expenditure rate. To date, Ascencia has spent a total of $106,674.17, leaving a balance of $284,439.83. In order to fully utilize the HHIP funds, Ascencia is requesting a no-cost extension to the existing contracts between the City of Glendale and Health Net, LLC, and City of Glendale and L.A. Care, through August 31, 2025. Health Net, LLC and L.A. Care are agreeable to the request.
City staff will continue to work with Ascencia to ensure timely expenditures and reporting of the HHIP program in order to close out the grant by August 31, 2025.

City staff and representatives from the Managed Care Plans (MCPs) provided a presentation to the CoC Board on September 13, 2022, and the presentation and the HHIP investment plan is posted on the Homeless Services homepage for public comment and feedback.
FISCAL IMPACT
There is no fiscal impact associated with extending the agreements. The HHIP funds were appropriated as part of the FY2024-2025 budget process; therefore, no appropriation is requested at this time.
