COUNCIL ACTION
Item Type: Action Item
Approved for March 31, 2026 calendar
EXECUTIVE SUMMARY
The Glendale Fire Department has participated in the State of California’s Public Provider Ground Emergency Transportation Inter-Governmental Transfer (PP-GEMT- IGT) Program since 2023. The program allows public ambulance providers to receive enhanced Medi-Cal reimbursement through federal matching funds administered by the California Department of Health Care Services (DHCS). Since implementation, Glendale has experienced higher-than-anticipated reimbursement revenue from Medi- Cal transports, resulting in increased quarterly IGT invoices from DHCS. Although the required local matching contributions have increased, the program continues to generate significant net revenue for the City. This report requests an increased appropriation to ensure sufficient funding is available to pay the quarterly IGT invoices, allowing the City to continue participating in the program and receiving the associated federal reimbursement revenue.

RECOMMENDATION
That the City Council approve an increased appropriation to cover the City’s required Inter-Governmental Transfer (IGT) payments associated with the PP-GEMT-IGT Program administered by the California Department of Health Care Services.
ANALYSIS
Since joining the PP-GEMT-IGT Program, the Glendale Fire Department has received substantially increased Medi-Cal reimbursement revenue associated with ambulance transport. Because the State calculates each agency’s required contribution based on the actual reimbursement revenue generated, Glendale’s quarterly IGT invoices have increased as program revenue has increased.
While the IGT payments have grown, the program continues to produce substantial net positive revenue to the City after all required contributions, administrative fees, and billing costs. Due to increased Medi-Cal transport revenue, the quarterly IGT invoices from DHCS have increased beyond the amount originally appropriated. Staff is therefore requesting appropriation to ensure adequate funding authority exists to pay the State invoices while continuing to receive the reimbursement revenue that offsets associated expenditures
To maintain participation in the program and continue receiving enhanced Medi-Cal reimbursements, the City must ensure sufficient budget appropriation authority exists to pay the required quarterly IGT invoices.

FISCAL IMPACT
We anticipate the IGT invoices will total approximately $3,100,000 for this Fiscal Year, of which $1,943,633 was included in the FY 2025-26 approved budget. The additional cost of $1,156,367, which will be offset entirely by the revenue received from the program, is being requested from the Undesignated General Fund Balance.
The increase in revenue has already been accounted for as part of the second quarter General Fund Financial Update that were presented to the Council on March 10, 2026, while the additional costs was not estimated at the time of the update. The additional annual costs of $1,156,367 will be included in the next General Fund Financial Forecast resulting in an annual decrease to future Fund Balance.
