EXECUTIVE SUMMARY
The City of Glendale requires specialized occupational medical services to ensure that its workforce, particularly public safety and field personnel, is medically fit for duty and compliant with state and federal regulations. The City has utilized Glendale Adventist Occupational Medicine Center (GAOMC) for these services since 2009. This report recommends renewing the Professional Services Agreement (PSA) with GAOMC for a five-year term with two one-year options to extend (2026–2033) to maintain service continuity and high-quality medical oversight for City employees.
The proposed agreement includes pre-placement physical examinations, regulatory medical examinations, drug and alcohol testing, fitness-for-duty evaluations, return-to- work assessments, and other occupational medical services necessary to support employee health, safety, and regulatory compliance. The annual not-to-exceed amount for these services is $200,000.

RECOMMENDATION
Authorize the City Manager to execute a PSA with GAOMC dba Adventist Health Glendale to provide occupational medical services including, but not limited to, pre- placement physical examinations, regulatory examinations mandated by Cal-OSHA, return-to-work evaluations, and fitness for duty examinations for the City employees for a five year term beginning November 1, 2026, with two one-year options to extend, and with an annual not-to-exceed amount of $200,000 (subject to annual appropriation).
ANALYSIS
In 2009, the City transitioned from an in-house employee health model to a contracted occupational medicine model following a competitive Request for Proposals (RFP) process. Glendale Adventist Occupational Medicine Center (GAOMC) was selected based on its specialized expertise in occupational health services and its ability to meet the City's public-sector workforce needs. In 2021, City Council approved a subsequent agreement extending these services. Throughout the current contract term, GAOMC has consistently provided reliable medical evaluations and occupational health services that support employee safety, workforce readiness, and regulatory compliance. The current agreement expires on October 31, 2026. Staff recommend a new five-year agreement with two one-year options to extend, which could continue services through October 31, 2033.
The scope of the proposed agreement ensures employees are medically fit for employment and maintains compliance with state and federal mandates. The services include, but are not limited to:
Pre-placement physicals (sworn and non-sworn) consistent with P.O.S.T. and City standards.
Mandatory regulatory and periodic examinations to meet DOT and Cal/OSHA and ongoing department compliance requirements.
DOT-qualified drug & alcohol testing, chain of custody, and Medical Review Officer (MRO) coordination.
Pre-employment and promotional, random, and reasonable-suspicion testing. Fitness-for-duty and return-to-work evaluations with written clinical recommendations.
Workers’ compensation medical evaluations and treatment coordination. Urgent occupational medical evaluation support as needed. Timely, secure transmission of medical reports and monthly/quarterly utilization and compliance reports.
Occupational medicine is a highly specialized field requiring expertise in employment- related medical evaluations, regulatory compliance, and safety-sensitive positions. 5e3

GAOMC is uniquely qualified to serve the City's workforce, including Police, Fire, and other employees whose duties require specialized medical screening and ongoing fitness-for-duty assessments. Continuing this long-standing partnership will preserve institutional knowledge, maintain service continuity, and ensure uninterrupted compliance with applicable health and safety regulations.
GAOMC's staff possesses extensive knowledge of state regulatory requirements, including Peace Officer Standards and Training (P.O.S.T.) medical standards, allowing examinations to be completed efficiently and in accordance with academy and certification timelines. Additionally, the clinic's location at 600 S. Glendale Avenue provides convenient access for employees, reducing travel time and minimizing operational disruption when medical evaluations or treatment are required.
Staff does not anticipate any significant changes to GAOMC's fee schedule during the contract term. Based on preliminary discussions, service rates are expected to remain substantially consistent with current pricing, allowing the annual not-to-exceed amount to remain at $200,000 throughout the initial term and any option years. This stability provides cost predictability and reflects the value of the City's long-standing partnership with GAOMC.

ALTERNATIVES TO STAFF RECOMMENDATION
1. The City Council may direct staff to conduct a competitive solicitation for occupational medical services. While this approach could provide an opportunity to evaluate alternative service providers, it may also result in a transition period requiring the transfer of institutional knowledge and implementation of new service delivery processes.
2. The City Council may approve a shorter-term agreement. While this option would reduce the contract term commitment, it would provide less long-term cost predictability and require staff to return to Council sooner for future contract authorization.
