_______________________________ __________ ____________ I am the Community Health Worker checking out this device. I agree to take full responsibility for the VIDA Wi-Fi Hotspot kit, including care and appropriate use of the equipment. By signing this document, I accept the following terms of use:
1. I work with the organization listed above.
2. I understand that the loan for the Hotspot kit is through March 30, 2022, and I need to return the Hotspot on or before the specified date. I cannot renew the Hotspot.
3. I will return the Hotspot to my organization CHW Coordinator.
4. If I return the device 2 weeks late, I will be charged the full replacement cost of the Hotspot Kit which is $50.
5. I will not use or let anyone use the Hotspot device for any illegal activity including:
transmitting material harmful to minors, harassing others online or downloading copyrighted material. I understand that illegal use of this device may be punishable by law.
VIDA will not be held liable.
6. I understand that Wi-Fi connections are not guaranteed to be secure. I accept responsibility for any information, including personally identifiable information I transmit while using this device.
7. I will notify my CHW Coordinator immediately if I have lost the Hotspot device.

Acepto la responsabilidad total del kit de Hotspot Wi-Fi de VIDA, incluyendo el cuidado y el uso adecuado del equipo. Al firmar este documento, acepto los siguientes términos de uso:
1. Soy trabajo con la organización mencionada anteriormente.
2. Entiendo que el préstamo para el kit Hotspot es el 30 de marzo de 2022 y necesito regresar el Hotspot en la fecha especificada o antes. No puedo renovar el Hotspot.
Devolveré el Hotspot al coordinador de CHW de mi organización.
3. Si regreso el dispositivo 2 semanas tarde, se me cobrará el costo total de reemplazo del Hotspot kit, que es de $50.
4.
