PROPOSAL
L.A. CADA will work with the City staff to develop an approved Operations and Service Plan for the site that outlines how the site will be operated, including but not limited to site intake, on-site services, procurement of meals and security, and ensuring that all residents receive the services that they need. The Plan will also operationalize how the site can maximize participant and program success by balancing and coordinating on-site services with existing services available through the County of Los Angeles, and the City's existing services contracts and resources. L.A. CADA will work individually with the Cohmis and city staff to learn the specific needs of the program participants and the areas of focus the city would like to prioritize. We understand that each city in Los Angeles County that is trying to make an impact in this housing crisis, has differing resources; as such L.A. CADA will tailor the delivery of proposed supplemental homeless outreach and services for the needs of the City of Hawthorne, working closely with city staff and to ensure that local needs are fully met and services are not duplicative. Additionally, L.A. CADA has developed an extensive and comprehensive list of resources available in SPA 8 with over 115 service providers and resources that will be useful to the program pmiicipants and ensure the success of the program in SPA 8.

We anticipate the need for urgent linkages to services (specialized case manager, mental health clinician, and SUD counselor) L.A. CADA staff will work with the selected pmtners on this program and deliver: Housing Navigation; Case Management; Homeless Prevention/Problem-Solving. L.A. CADA will also provide Supportive Services (referral to legal services, workforce development, etc.); Mental Health Support; Health Support; and direct funding resources. L.A. CADA will identify participant's existing services providers.

If one does not exist, the participant will be enrolled into either L.A. CAD A's continuum of care whenever possible, or the most appropriate provider. The Hawthorne Homeless Service Center will be leveraged where possible to ensure continuity of services with housing navigation. For pa1ticipant's linked to another provider, L.A. CADA will ensure a warm hand off is completed. Additionally, these participants will have regular/consistent followups with the outside provider to ensure client/participants are making progress towards established housing plans and case management goals. L.A. CADA will provide support as needed to assist in patticipant's achieving these plans and goals. For those with an external provider, monthly care coordination with said provider will be conducted. For pmticipant's enrolled into services, housing plans and case management goals will be established within no more than two weeks of admission. Subsequently, all housing plans will be reviewed and re-assessed every 90 days. Participants will have weekly case management sessions, which will include any and all required housing navigation services. Priority will be given to medical and mental health needs.

L.A. CADA will accomplish the following project tasks:

Service 1: Develop Operations and Services Plan
9 Original Agreement
for Reference

Task 1.2: Services Coordination. L.A. CADA will Identify and meet with existing service provider paitners to determine opp01tunities and approaches to provide services for residents at the site. Approaches could include but are not limited to transport to off-site services or regularly-scheduled visits to the site. Staff from the City should be included in these meetings.

Task 1.3: Plan Development.

L.A. CADA will work with the City staff to develop an Operations and Services Plan (Plan) for the site. The Plan should outline how the site will be operated to ensure that all site residents receive the resources that they need to move into permanent housing as quickly as practicable. The Plan should operationalize how the Consultant will work with stakeholders to maximize pmticipant and program success by balancing on-site services with existing services available through the County of Los Angeles, and the City's existing services contracts and resources, which are described in more detail in the Project Description. The Plan should outline roles and responsibilities for onsite staff, as well as how these staff will work with other offsite staff and services. The Plan should also outline the approach to ensure that all needed services are available for the site's patticipants, and for services not available at the site, where the services can be found and how they would be accessed. For example, the Plan should maximize pmticipant connection to these existing services and strengthen coordination with pmticipants existing case managers and housing navigators.

Considerations that should be included in the Plan include but are not limited to the following:
1) Site policies and procedures;
a) Eligibility & referrals, program intake, and program exit; 2) Site management and oversight,
a) Coordination with other on-site services ( e.g. security; meal delivery); 3) Case management, housing navigation, and connection to existing services; On-site service availability;
a) Approaches to maximizing coordination with existing and off-site services; 4) Staffing levels, qualifications, and schedule

10 Service 2: Operate Site and Provide Services
TASK 2.1: Site Operation
The Provider will operate the site for a minimum of three years in accordance with the Final approved Operations and Services Plan. L.A. CADA will staff at minimum the follow three positions for this project:
• 1 FTE on-duty during weekday business hours to oversee site (facilitate meals, showers, etc.), conduct program intake and exit, provide case management as needed, coordinate with participants' existing case managers and housing navigators, and connect participants to existing services as needed.
• 2 FTE resident aides on-duty weekday evenings and weekends to oversee site (facilitate meals, showers, etc.).
• 1 FTE maintenance staff persons on duty weekday evenings and weekends to keep the site clean and operational.
• 1 FTE Housing Navigator with substantial experience in connecting program participants with permanent housing to ensure the success of the program. • 1 FTE Case Managers to ensure close collaboration between services and efficient and fruitful linkages to care, and who will work closely with the Housing Navigator to find appropriate housing for all program participants.

These staffing levels may be modified in accordance with the final approved Operations and Services Plan. L.A. CADA will also be providing a monthly rep01t in a format approved by the City, demonstrating aggregate enrollment and exit data and provision of/connection to services.

The proposed staffing plan could be modified based on the Operations and Service Plan developed in Task 1. Deliverables: Monthly Repmts (12)
TASK 2.2: Project Management
The Consultant will host a kick-off meeting and monthly check-in calls with the City.

Deliverables: Kick-off meeting agenda and meeting notes (1 ); monthly meeting agendas and meeting notes (12).

L.A. CADA will establish MOUs with our community partners to detail procedures and responsibilities for the coordination of services and refetTals. These paitners will include:

SPA 8 CES; MultidisciplinmJI Teams (MDT); Housing Navigation Teams; PubUc Spaces Teams; the LAHSA Homeless Engagement Teams (HET); as well as local providers for Supplemental Homeless Services and Services for Hard-to-Reach Populations.

Additionally, L.A. CADA is skilled and experienced at working with city staff, local law enforcement, and community service providers to coordinate service referrals and program services.

Existing L.A. CADA partnerships will be useful to the proposed program. This includes our partnership with the Los Angeles Mission (where we house up to 50 chronically homeless 11 men with SUD or cooccurring SUD and mental health disorders (COD). L.A. CADA also has a strong partnership with Homeboy Industries (where we provide SUD and COD treatment, as well as transitional supportive housing services for Homeboy trainees (new hires). We also pmtner with Transformational Living Homes (where we screen and provide suitable housing for low-to-medium risk Registered Sex Offenders with SUD or cooccurring SUD and COD).

L.A. CADA is the designated SUD service provider for this housing program. This will allow for timely and efficient linkages to SUD, mental health, MAT, and various other services we offer or deemed necessary through the length of the project. We are currently offering Homeless outreach services to the city of Hawthorne and have developed an effective and positive working relationship with the City staff and an in depth understanding of the area. Through our Hawthorne homeless outreach work MOU, we are already collaborating with St. Margaret's Center (Catholic Charities) who is one of the city specific providers for this program and have developed a positive working relationship with them.

L.A. CADA is a long- time participant in the L.A. County Coordinated Entry System - 95% of our residential treatment clients are homeless at service intake. We participate in the CES in SP As 4, 7, 8, and 3. In addition, our agency is a CES Access Point in SPA 7. L.A. CADA is familiar with CES procedures/resources, including use of the CES Triage Tool, the Vulnerability Index Service Prioritization Decision Assistance Tool (VI-SPDA T), and CES reporting requirements. For the proposed project, L.A. CADA will designate staff to attend SPA 8 CES meetings. Our project staff will work closely with local Access Centers and Access Points in both SP As to accept referrals of homeless individuals in need of L.A.

CADA services. This will include service coordination with Union Station Homeless Services (serving adults) and Sycamores (serving TAY). L.A. CADA staff will facilitate warm hand-offs to needed services for homeless residents in Hawthorne via L.A. CADA van/driver. Referrals to resources will be facilitated by formal Memorandum of Understanding (MOU) that L.A. CADA will put in place with project pa1tners in the community.

L.A. CADA will leverage our own considerable resources for this project. Homeless residents of Hawthorne served will have priority access to:
248 state-licensed residential treatment beds (including a 16-bed 24/7 mental health crisis treatment program in Santa Fe Springs);
244 Recovery Bridge Housing beds (including 71 beds in the San Gabriel Valley); Over 500 state-ce1tified outpatient treatment slots; and A new 20-bed emergency shelter to be opened before Summer 2022.

The City of Hawthorne will work directly with L.A. CADA team members to access the proposed services in the manner that best suits that city. L.A. CADA project staff will schedule regular meetings with city staff, and communicate frequently via phone, email, and official reporting documents to ensure that the project is filling the needs of the city.

L.A. CADA will also provide a 24/7 response number so that city staff can communicate with L.A. CADA project staff at all times.
12 L.A. CADA can also ensure the success of the site by referencing our expertise with this service population, our ability to leverage our own services, and our adherence to quality assurance. Our primary service groups are vulnerable, disadvantaged populations. These groups include: the homeless; low-income residents; persons of color; pregnant/parenting women and their children; persons living with and at-risk of HIV; high-risk youth and their families; people involved in the criminal justice system; and LGBTQ+ persons. L.A. CADA is one of the only behavioral healthcare providers in Los Angeles County able to deliver LGBTQ+ Affirming (exclusive) care - rated the highest quality of treatment for this population by the federal Substance Abuse and Mental Health Services Administration (SAMHSA). 3
In 2022, the L.A. CADA continuum of care includes over 500 slots for outpatient and intensive outpatient treatment, 248 residential treatment beds, 244 Recovery Bridge Housing beds, and a broad spectrum of jailbased, court-based, school-based, and community-based services. As noted, L.A. CADA facilities include a 16-bed Crisis Residential Treatment Program (CRTP) called Safe Haven, funded by the L.A. County Department of Mental Health to stabilize adults in mental health crisis. Client paiticipation in this short-term program can be followed by residential or outpatient treatment at L.A.

CADA or in our Residential Bridge Housing (RBH) with off-site outpatient care. RBH provides a safe place for people in behavioral health crisis to stabilize and establish early sobriety. In 2021 L.A. CADA opened a Certified Community Behavioral Health Clinic (CCBHC) in Santa Fe Springs to deliver comprehensive, coordinated care for low-income people. Our treatment facilities are ce1tified and licensed by the State of California Department of Health Care Services, and monitored by the Los Angeles County Department of Public Health.

Los Angeles Centers for Alcohol and Drug Abuse maintains a full-time Quality Improvement (QI) Division to evaluate our services and their utilization by our patients. For each funded project, an evaluation plan is developed, including a Logic Model detailing specific goals, objectives, inputs and expected outcomes. Our QI Coordinator works with program staff to continuously collect, monitor, review, and report diverse client level data:
demographics; histories (substance use, mental health, treatment, housing, family issues, social services), as well as client treatment paiticipation and outcomes.

Data collection is conducted using both pencil/paper documents and the agency's electronic records system (SAGE). Staff enter all required project and program data on a daily basis.

Files are reviewed weekly by each Program Director/Manager to ensure completeness and accuracy. During monthly meetings with the Coordinator of Quality Improvement, project staff discuss and review progress toward program goals and objectives. Any needed course corrections to improve the program are implemented by the Program Director/Manager at this time.

EXHIBIT B
Scope of Services
L.A. CADA will work individually with the Cohmts and city staff to learn the specific needs of the program participants and the areas of focus the city would like to prioritize. CADA has developed an extensive and comprehensive list of resources available in SPA 8 with over 115 service providers and resources that will be useful to the program pmticipants and ensure the success of the program in SPA 8.

We anticipate the need for urgent linkages to services (specialized case manager, mental health clinician, and SUD counselor) L.A. CADA staff will work with the selected partners on this program and deliver: Housing Navigation; Case Management; Homeless Prevention/Problem-Solvh1g. L.A. CADA will alsoprovide Supportive Services (referral to legal services, workforce development, etc.); lvlental Health Support,· Health Support; and direct funding resources. L.A. CADA will identify participant's existing services providers.

If one does not exist, the participant will be enrolled into either L.A. CAD A's continuum of care whenever possible, or themost appropriate provider. The HawthorneHomeless Service Center will be leveraged where possible to ensure continuity of services with housing navigation. For pmticipant's linked to another provider, L.A. For pmticipant's enrolled into services, housing plans and case management goals will be established within no more than two weeks ofadmission.

Task 1: Project Management. LA CADA will conduct Kick Off meetings with the City of Hawthorne to introduce our agency, listen to stakeholder needs, and to review the project scope of phase I and 2, schedule, goals, and key issues.

Task 1.1: Kick Off Meetings. L.A. CADA will provide key Task 1 deliverables of a meeting agenda, and meeting notes.

Task 1.2: Services Coordination. L.A. CADA will Identify and meet with existing service provider pminers to determine oppo1iunities and approaches to provide services for residents at the site.

The Plan should operationalize how the Consultant will work with stakeholders to maximize participant and program success by balancing on-site services with existing services available through the County of Los Angeles, and the City's existing services contracts and resources, which are described in more detail in the Project Description. The Plan should outline roles and responsibilities for onsite staff, as well as how these staff will work with other offsite staff and services. The Plan should also outline the approach to ensure that all needed services are available for the site's participants, and for services not available at the site, where the services can be found and how they would be accessed. For example, the Plan should maximize pmticipant connection to these existing services and strengthen coordination with pmiicipants existing case managers and housing navigators.

Considerations that should be included in the Plan include but are not limited to the following:
1) Site policies and procedures;
a) Eligibility & referrals, program intake, and program exit; 2) Site management and oversight,
a) Coordination with other on-site services ( e.g. security; meal delivery); 3) Case management, housing navigation, and connection to existing services; On-site service availability;
a) Approaches to maximizing coordination with existing and off-site services; 4) Staffing levels, qualifications, and schedule
5) Support securing additional funding and resources
a) Approaches to maximizing operational funding, including but not limited to coordination with government programs for which participants are eligible (e.g.

Medicaid/Medi-Cal, Social Security Disability Insurance [SSDI]) b) Facilitating support and donations from community members and groups; and 6) Other site considerations.

Service 2: Operate Site and Provide Services
• 1 FTE Case Manager to ensure close collaboration between services and efficient and fruitful linkages to care, and who will work closely with the Housing Navigator to find appropriate housing for all program participants.

This includes our partnership with the Los Angeles Mission (where we house up to 50 chronically homeless men with SUD or cooccurring SUD and mental health disorders (COD). L.A. CADA also has a strong partnership with Homeboy Industries (where we provide SUD and COD treatment, as well as transitional supportive housing services for Homeboy trainees (new hires). We also paitner with Transformational Living Homes (where we screen and provide suitable housing for low-to-medium risk Registered Sex Offenders with SUD or cooccurring SUD and COD).

L.A. CADA is a long- time paiticipant in the L.A. County Coordinated Entry System 95% of our residential treatment clients are homeless at service intake. We participate in the CES in SPAs 4, 7, 8, and 3. In addition, our agency is a CES Access Point in SPA 7. L.A. CADA is familiar with CES procedures/resources, including use of the CES Triage Tool, the Vulnerability Index Service Prioritization Decision Assistance Tool (Vl-SPDAT), and CES reporting requirements.

CADA will put in place with project paitners in the community.

L.A. CADA will leverage our own considerable resources for this project. Homeless residents of Hawthorne served will have priority access to:
248 state-licensed residential treatment beds (including a 16-bed 24/7 mental health crisis treatment program in Santa Fe Springs);
244 Recovery Bridge Housing beds (including 71 beds in the San Gabriel Valley); Over 500 state-certified outpatient treatment slots; and A new 20-bed emergency shelter to be opened before Summer 2022.

L.A. CADA can also ensure the success of the site by referencing our expertise with this service population, our ability to leverage our own services, and our adherence to quality assurance. Our primary service groups are vulnerable, disadvantaged populations. These groups include: the homeless; low-income residents; persons of color; pregnant/parenting women and their children,· persons living with and at-risk of HIV; high-risk youth and their families; people involved h1 the crirninaljustice system; and LGBTQ+ persons.
Client pmticipation in this short-term program can be followed by residential or outpatient treatment at L.A.

Our QI Coordinator works with program staff to continuously collect, monitor, review, and report diverse client level data:
demographics; histories (substance use, mental health, treatment, housing, family issues, social services), as well as client treatment pmticipation and outcomes.

As noted throughout the application L.A. CADA will leverage our own resources which includes SAPC, DMH, SAMHSA, and other government contracts to secure additional support and funding for services available to the site and the needs of the program participants. L.A. CADA programs work closely with the L.A. County Coordinated Entry System (CES) for the homeless in the Service Planning Areas (SPAs) that L.A. CADA serves (SPAs 3; 4; 6; 7; and 8). As such, we have experience coordinating additional services, if and when necessary, through the SPA 8 CES
