Recommendation toauthorize theCityAttorneytosubmitStipulationswith Request for Award forapprovalby theWorkers’Compensation Appeals Board, and ifso approved, authoritytopay 1 )$11,092.50and 2)separateauthorityto pay $8,772.50,and 3) separateauthoritytopay $58,290.00incompliancewith the Appeals Board Order for Cleo Lee.
DISCUSSION:
Cleo Lee has made a claim for injuriesallegedlysustainedin the course of his employment with the City of Long Beach. The Government Operations and Efficiency Committee on July 18, 2023, approved the submission to the Workers’ Compensation Appeals Board of threeStipulationswithRequest forAward by the City and Cleo Lee, based upon 1)a 12 percentdisabilityratingwithprovisionfor lifetimemedical may be necessary,and 2) a 10 percentdisabilityratingwith provisionfor lifetimemedical care as may be necessary,and 3) a 40 percentdisabilityratingwith provisionfor lifetime medical care as may be necessary.
