Burke Ham Park - Field Visit
Chris Fitch <[email protected]> Fri, Dec 3, 2021 at 7:34 AM To: "David A. Santana AIA" <[email protected]> Hello David,
Please see the attached and below notes:
Image 4568 shows the area where a few conduits meet together that leads me to have concerns about the contractor maintaining the required per plans 18” minimum burial depth.
Image 4578 shows the not yet installed as required light near the sign.
Image 4581 shows the pole lights mounting being off center. The cover’s for the bolting are not well attached.
Image 4611 shows the unfinished connection back to the pole north, I believe that it terminated here as the feeders were cut off here.
They seemed to rather than do complete straight runs they had points where they went up to intercept rather than going directly to locations, not a major issue if they maintained requirements.
Thank You,
Christopher Fitch

3. Irrigation- unknown if a specification required preinstallation conference was held. Drip line system is missing from all shrub planted areas. Unknown how shrubs are being watered. Unable to view spray heads due to overgrowth of turf. Lateral lines were not reviewed prior to burial, so depth of lines has not been verified. Mainline not placed per irrigation details. Water pressure needs to be verified. Unknown if a coverage test has taken place, or if irrigation heads are set at the correct height.
4. Planting- unknown if a specification required preinstallation conference was held. Four (4) Crape Myrtle trees were observed to have died. It appears that the four trees that died are a different variety than the surviving Crape Myrtle trees. Trees shall be replaced in-kind as noted in the specifications. An unknown quantity of shrubs were observed to be in poor health or dead. A specification required repair and replacement program has not taken place. Mulch is not in-place as required by the plans and specifications.
Turf has not been mowed and is approximately 8"-12" in height. A specification required weed abatement operation has not been enforced.
b. Concrete play area curbs- curbs appear to be incorrectly constructed with no expansion joints and need additional review by the civil engineer. North east corner is incomplete. Refer to civil comments for additional information. Unknown if a preinstallation conference was held. c. Concrete mowstrips- portions of mowstrips are incorrectly constructed with no expansion joints and incorrectly located. Refer to civil comments.
d. Shade structures- not constructed
e. Tubular steel fencing planned for construction at back of walk, adjacent to the parking lot, has not been constructed.
f. Site furniture- benches, trash receptacles and tree grates with concrete bands are not in-place. g. 5-12 play area equipment differs from play equipment footprint shown on plans. Verify if equipment in the 5-12 area is per plans and specifications, and determine if res pective safety zones are per current CPSC standards.
Refer to the attached supplemental site photos pertaining to items noted above.

General Comments
Weed debris needs to be cleared, however, the vegetation looks healthy. Nevertheless, little progress has been made since the 07.12.21 Field Report. We remain concerned that the project will not be completed by 09.15.21. A Project construction schedule needs to indicate construction tasks now starting from 08.01.21 to project completion.
In addition, the pending construction schedule needs to indicate the next major task which is the installation of the shade structure. Installation of sod and the coordination of the shade structure installation should be provided for City concurrence.

Estimated Completion of Construction: 35% (REVISED)
Work Observed in Progress:
1. Two field laborers were on site working with shoves and moving soil at Southwest area of site
2. No other construction activity was evident during Field Report Observations:
1. It appears that weed growth is overtaking ground cover and newly placed plant material s
2. New trees appear to be healthy and thriving
3. Some areas appeared to be cleared of weeds and are being prepared for landscape installation
4. Progression of footing for new archway pillar has remained stagnant with irrigation line requiring relocation
5. Project site debris requires efforts to clear the site
6. The temporary construction fence remains incomplete
Action Items/Areas of Concern
1. Weed clean-up is required
2. Key construction items should be started:
A. Three (3) new tree wells at southeast site area
B. New entry archway with CMU pillar
C. New west site perimeter fencing
D. New interior light standard
E. New concrete for northeast shade structure placement
3. Completion of temporary construction fence
General Comments
Project construction appears stagnant with very little progression since July field reports. There are rarely any construction crew laborers on site during our field visits, although two were evident recently. There is substantial work required to complete this project. We don't believe the project will be completed by September 15, 2021.
We are indicating a (REVISED) 35% completion status of the project considering that substantial items have not been addressed including, the new interior light standard, the new drinking fountain, the two bench shade structures, the concrete work at the northeast and the southwest corner s of the site to accommodate the bench shade structures and three new trees, respectively. However, the most significant scope of work remains, including the new playground shade structure, the playground equipment and the playground safety surface.

Estimated Completion of Construction: 40%
Work Observed in Progress:
1. No construction crew or laborer activity was evident during Field Report
2. Sod has been recently installed.
Observations:
1. It appears sod quantity was excessive
2. Sod was placed in location where future vertical shade structure support will be installed
3. Progression of footing for new archway pillar has remained stagnant with irrigation line requiring relocation
4. A homeless individual has established residency on the site
5. T emporary construction fence remains incomplete. It is compromised at two locations allowing unauthorized entry Action Items/Areas of Concern
1. Weed clean-up is required at recent planted areas
2. Excessive sod material should be returned to vendor for City credit
3. Completion of temporary construction fence; this continues to be an unresolved matter allowing unauthorized entry
4. Remove the unauthorized homeless individual from the construction site General Comments
Installation of sod has been completed. However, when the installation of the new shade structure for the existing playground is taking place, areas of the new ly placed sod will be affected. It appears that the placement of the sod does not align with an appropriate sequence of construction tasks. It is our belief that sod should had been one of the last tasks to be completed. However, a current construction schedule remains an action item for the General Contractor.
Moreover, it appears that the quantity of sod material was over estimated by the General Contractor. Therefore, they should return unnecessary sod materials to the sod vendor in order to provide the City with a credit for unused materials.
Lastly, with multiple compromised entry points at the perimeter temporary construction fence , unauthorized homeless individuals have penetrated the construction site and may have put the City of Lynwood in a potential liability state. Therefore the General Contractor should remedy the situation as soon as possible.

Work Observed in Progress:
1. Three laborers were on site
2. It appeared that work related to playground equipment placement and safety zones were being designated Observations:
1. Sod appears to be healthy
2. The placement of three new trees in grated areas were installed, however not to plans and specifications
3. Playground equipment could not be visualized, however trashed pallets suggest equipment might be stored onsite
4. The homeless individual has been removed from the construction site
5. Temporary construction fence remains incomplete. It remains compromised at two locations as previously reported Action Items/Areas of Concern
1. The General Contractor needs to provide a Change Order CREDIT for concrete pavement not provided
2. GC should provide confirmation of playground equipment on site
3. Completion of temporary construction fence; this continues to be an unresolved matter allowing unauthorized entry General Comments
Work on site appears to reflect placement of playground equipment. However, physical evidence of equipment could not be verified on site. Excessive trash on site suggests that pallets carrying playground equipment may be stored in the "WHITE" storage container.
The placement of the three trees at the adjacent walkway have been placed. However, it appears that the General Contractor saw-cut existing concrete and placed the three new trees. This application does not meet the plans and specification of the project. Therefore, the General Contractor should provide the City with a Change Order "CREDIT" for the lack of new concrete pavement.
Please see attached bid document indicating the area of the three new trees described in this Field Report.

Conditions:
60 degrees / Clear with sunshine
Estimated Completion of Construction: 55%
Work Observed in Progress:
1. No laborers were on site
Observations:
1. Temporary construction fence remains incomplete. Gate remains unchained and opened as previously reported
2. New playground equipment appears to be fully assembled, but without concrete footings in place
3. New CMU pillar for the new entry archway appears to be fully grouted without placement of the metal archway
4. A portion of the steel metal archway was evident on site. It requires substantial preparation for paint finish
5. Substantial vegetation appears to be withering; one tree appeared damaged and horizontal at the site
6. Concrete for the future seat shade structures has been poured
7. Swing set playground equipment has been initiated
8. The site needs to be cleared, cleaned and have debris removal coordinated as per bid documents Action Items/Areas of Concern
1. The General Contractor did not follow the 08.12.21 archway details and has filled the CMU pillar incorrectly
2. GC should straighten the new playground equipment and pour concrete footings
3. Shade structure construction tasks need to start
4. The Temporary construction fence gate remains unchained and opened and places the City in a compromised position General Comments
Some vegetation appears to be withering and unhealthy. Watering should be discussed to maintain the existing vegetation in a thriving fashion. One tree appears to have been knocked down and broken.
The General Contractor has not followed the specifications, construction documents and the 08.12.21 archway clarification details for the new entry archway. There does not appear to be space for a CMU cap piece as the design drawings indicated. This field report provides a partial detail image with t he integral base displayed surrounded by the CMU cap pieces. The site is covered with debris, trash and various mounds of soil. The GC needs to follow the bid documents as it pertains to maintaining a clean construction site.
Lastly, we remain concerned that the temporary construction fencing is unlocked, unattended and open during the day. This situation could put the City in a compromised and potential liability position if an unsupervised youth decides to climb the new playground equipment. The GC should have a lock and chain at the construction site entrance for safety measures.

Estimated Completion of Construction: 55%
Work Observed in Progress:
1. No laborers were on site
Observations:
1. The temporary construction fence remains incomplete. Gate remains unchained and opened as previously reported
2. New playground equipment footing have been poured, however the quality of concrete appears less than acceptable
3. Several landscape elements appear to be dying and require irrigation
4. Weeds have overcome most of the planted areas and are threatening the well-being of planted materials
5. Swing set playground equipment remains incomplete
8. The site needs to be cleared, cleaned and have debris removal coordinated as per bid documents Action Items/Areas of Concern
1. The General Contractor needs to address the site in the following manner:
A. Clear the site of unnecessary trash items and construction debris per Project Specifications B. Replace t
he security netting surrounding the construction site temporary fencing C. Secure the construction site with a lock at the entry gate to eliminate potential unauthorized personnel on site
2. Shade structure construction tasks need to start
3. The
City should consider investigating the strength of the concrete mix poured for the playground equipment and obtain
written confirmation and documentation from the General Contractor that the concrete mix meets Project Specifications a
nd minimum playground equipment requirements.
General Comments
Some vegetation appears to be w ithering and unhealthy. Watering shoul d be di scussed to maintain the e xisting vegetation in a thriving fashion. Several trees appear to be dying, and irrigation is required.
From a visional inspection, it does not appear that the General Contractor follow the Project Specifications as it pertains to the Cast in place concrete requirements. The City should request written confirmation and concrete mix certification from the General Contractor, or consider demanding the removal of the concrete and having the Ge
neral Contractor follow the specifications for the concrete pour of the Playground Equipment component to ensure full safety measures have taken place.
Lastly, we remain concerned that the temporary construction fencing is unlocked, unattended and open during the day.
This situation could put the City in a serious and potential compromised liability situation if an unsupervised youth decide to climb the new playground equipment. The GC should have a lock and chain at the construction site entrance for safety measures.
