Re: On Call Professional Surveying Services – Ellis Lake Topographic Survey Dear Mr. Schaad,
MHM is pleased to provide you with the following proposal for surveying services for the Ellis Lake Proposition 68 / Clean California grant and future projects around the lake.

As we understand the project, City of Marysville is requesting topographic surveying services for the project to repair and install new walkways around the lake and redo much of the open space areas around the lake. The new project walkways around the lake will include connecting to the existing streets, and conforming to existing grades and slopes. The work for MHM will require field surveys, post processing of data, professional supervision of horizontal and vertical control, terrain modeling and preparation of base mapping for the entire lake property. We have outlined below the task that we understand to be in our scope of work, information furnished to us, as well as tasks that would not be a part of this proposal.

INFORMATION TO BE FURNISHED TO MHM

1. A signed Purchase Order under our existing on-call Contract.

2. Any additional project requirements.

TASK 1 PROP 68 PROJECT COORDINATION FEE: $ 6,406.00
TASK 2 SITE SURVEYING AND BASE MAPPING FEE: $ 22,326.00 a. MHM will provide all work in order to complete topographic survey and base mapping for the project boundaries, as is adequate for the full design of the project. The work shall include all ground surveys, record data search and right- of-way map. The ground surveys shall include, but not be limited to the following; all curb, gutter, sidewalk, edge of pavement (or road), walls, structures, fencing, gates, trees with diameter greater than 4 inches, landscaping (including yard drainage directions), ditches, swales, culverts, grade breaks, any existing pavement striping or markings, storm drain manholes or inlets, sanitary sewer manholes or cleanouts (including dip elevations), water valves or meters, fire hydrants, gas valves, all underground or overhead utility facilities within the limits described below and all other topographic features needed for design.

Contour intervals will be one half foot. The site topography shall include all of the limits identified

on the City provided exhibit of the preliminary project. The width of the survey shall be from the existing fence line or 10’ into the adjacent road surface to the water surface for topographic survey.

In addition, the survey shall include a bathymetric survey of the lake bottom. Existing right of way boundaries and property lines will be included on the base map. Additionally, any grade breaks in the existing ground and quarter-crowns in travelled way will be included. The approximate limits of this survey are shown on the City provided Preliminary Site Plan and the survey shall generate adequate information for designing the facilities.
b. MHM will provide permanent horizontal and vertical survey control to be used for construction staking of the project in an AutoCAD DWG file compatible with Civil 3D 2014. c. MHM will also provide a Digital Terrain Model (DTM) in an XML file along with Topo Points.
d. MHM will continue surveying, without additional payments, until the City is satisfied that all topographic features needed for design are captured and included in the Topo Points.

INFORMATION OR TASKS NOT PROVIDED BY THIS PROPOSAL

1. Underground utility location of utilities that are not marked or identified by record drawings.

2. Engineering reports, inspection or design

3. Geotechnical Engineering investigation, boring or reports FEES
The fees shown for the work described as “Task 1 to be performed by MHM” would be billed on a time and materials basis based on the attached cost breakdown not to exceed the Lump Sum amount listed above. Statements will be coordinated with the regularly monthly billing cycle of the City of Marysville.

ADDITIONAL SERVICES
If additional services are required outside of the scope of work described, it would only be performed with written authorization as to scope and cost.

We can schedule the work immediately upon authorization and look forward to working with City of Marysville on this project. Please feel free to contact me if you have questions.

Sincerely,
John Mallen, P.E., P.L.S.
