This Change Order / Addendum becomes a part of and is to be attached to the Agreement made by and between Hoffman and Subscriber, and is referenced by the name of subscriber for additions or changes to the former Agreement. All provisions of the original Agreement apply with the same force and effect to this Change Order.
Hoffman to upgrade existing 3G cellular communicators to 4G LTE Cellular communicators at $250 each. Work to be done at Prevailing Wage Rates.

Additional Cost To Subscriber: Two hundred fifty dollars and No Cents... .. $250.00 ACCEPTANCE OF Change Order - The above prices, specifications and conditions are satisfactory and are hereby accepted. Hoffman is authorized to do the work as specified. Payment will be made as outlined above.
