Impact of Federal Budget Cuts
and the County’s Response
County of Santa Clara 1 of 15
H.R. 1 Overview
H.R. 1, or the “One Big Beautiful Bill Act,” is the federal budget reconciliation bill that was signed into law by President Trump on July 4, 2025.
Enacts the largest cuts in our nation’s history to Medicaid (known as Medi-Cal in CA) and food assistance (CalFresh in CA) to help fund tax cuts primarily benefitting the wealthiest Americans.
$1 trillion in cuts to Medicaid
$186 billion in cuts to Supplemental Nutrition Assistance Program (SNAP) Medicaid is a critical social safety net program that provides healthcare coverage for nearly 1 in 5 Americans. Approximately half of all children in the United States are covered by Medicaid.
SNAP provides basic food assistance for 1 in 8 Americans. SNAP is the largest child nutrition program providing benefits for 1 in 4 children in the United States. 2
2 of 15
H.R. 1 Impacts to Our Community
H.R. 1 puts access to healthcare and food assistance at risk for thousands of families in Santa Clara County.
1 in 4 Santa Clara County residents (465,000) rely on Medi-Cal. 133,000 Santa Clara County residents rely on CalFresh for food assistance. Counties in California have a legal obligation to provide care to the poor (Welfare and Institutions Code, Section 17000) and all hospitals are legally obligated to provide emergency care to patients regardless of the patient's ability to pay. With a much larger uninsured population and dramatically fewer resources with which to provide care, H.R. 1 puts the County's hospitals and health services at risk, threatening access to care for all residents.
3 3 of 15
H.R. 1 Creates a County Fiscal Crisis
Medicaid is the single largest source of federal funding for the County, totaling $2.3 billion in FY25-26.
Medi-Cal funding accounts for more than half of all revenue for Santa Clara Valley Healthcare (SCVH), the County's healthcare delivery system. The County expects $1 billion in lost revenue per year, representing a significant portion of the $4.6B total annual budget for Santa Clara Valley Healthcare This is a devastating and destabilizing level of cuts that can only result in extraordinarily harmful service reductions if not addressed.
4 4 of 15
Projected Impact to County Revenues (Summary)
5 of 15
Santa Clara Valley Healthcare operates 4 hospitals and 15 major health centers in Santa Clara County and is the largest provider of many critical healthcare services to all residents: Provides care for nearly 1 in 4 Santa Clara County residents. Operates 2 of only 3 trauma centers in Santa Clara County and handles 80% of all trauma cases originating within the county. Every 11 minutes a 911 call in Santa Clara County results in a patient being transported to a County hospital.
Largest provider of both Medi-Cal and Medicare hospital services to Santa Clara County residents.
Top ranked Rehabilitation Center on the entire West Coast. Only Burn Center in Santa Clara County and 1 of only 3 regional centers of its kind between Los Angeles and the Oregon border.
H.R. 1 Threatens Access to
Healthcare and Local Hospitals 6
6 of 15
Why Does the County Operate
Four Hospitals?
All 3 of the hospitals the County has acquired in recent years were purchased to prevent hospital closure or to
reverse the termination of critical lines of service
In 2019, the County was the only bidder when
O'Connor and St. Louise went through bankruptcy.
In 2025, the County purchased Regional Medical
Center (RMC) after the former operator terminated
trauma, STEMI, and other critical services in August
2024. SCVH restored these services on April 1.
For every dollar the County spends on healthcare, 88 cents comes from healthcare system revenue.
It is only due to the unprecedented cuts from the
federal government that the sustainability of these
hospitals has been put at serious risk.
7 of 15
H.R. 1 Puts County Services at Risk
8 of 15
Federal cuts of the magnitude we face present a major risk to all County services from public safety to homelessness. A $1 billion funding shortfall is too significant for Santa Clara Valley Healthcare to absorb on its own.
3,700+ residents in Monte Sereno and Los
Gatos rely on Medi-Cal
5,500 residents of Monte Sereno, Los Gatos,
and Saratoga receive care from Santa Clara
Valley Healthcare
More than 14,000 outpatient visits
Nearly 2,000 Emergency Room visits
3,000 hospital days

30% of children transported by ambulance are
taken to a County hospital
Investments in critical homelessness
prevention, shelter, and housing solutions

FOOD ASSISTANCE
750 residents in Monte Sereno and Los Gatos
rely on CalFresh to put food on the table
County’s Response to H.R. 1
To address the extraordinary fiscal emergency caused by H.R. 1, the County is taking swift, proactive steps in several areas. 10 Reducing Costs and Increasing Revenue
Seeking State Support
Asking Our Local Community for Help
10 of 15
County’s Response
Actions to Reduce Costs and Increase Revenue
The County has launched a multitude of initiatives in recent fiscal years in response to declining economy and slowing property tax growth:
Operational and organizational efficiencies
Restructuring programs and service delivery models
Reviewing fees and charges to ensure full-cost recovery Hiring freezes and staff reductions, including eliminating close to 800 positions over the past three fiscal years
Despite these actions, the funding gap from H.R. 1 is unprecedented.
Sizable reductions and consolidations of services will be needed. 11 11 of 15
County’s Response
State and Local Actions
The State will play a critical role in determining how federal cuts under H.R. 1 are absorbed.
Medi-Cal and CalFresh are jointly funded by the federal and state government.
County Administration will work closely with the State to protect public hospitals
Public hospitals make up only 6% of California’s hospitals but operate more than 50% of trauma and burn centers and train over 50% of California’s doctors.
State support is uncertain and not fully under the County’s control.
The County must look locally to avoid devastating service cuts.
12 12 of 15
On August 7, 2025, the Board of Supervisors unanimously voted to place an emergency, temporary general sales tax measure of 5/8 of a cent on the ballot.
If approved, the tax would be in place for five years and generate $330 million in revenue each year.
The County has very limited options for raising local revenue for services.
Unlike cities, counties can only enact taxes that have been affirmatively authorized by the State. SB 335 grants the County temporary authority to provide voters the option to raise local sales taxes by 5/8 of a cent for County services.
Measure A does not affect the ability of any city to propose a sales tax measure to their voters.
Under state law, common household expenses (e.g. rent, groceries, utilities, childcare, etc.) are exempt from sales taxes, reducing impact on families.

County’s Response
Measure A
13 of 15
Summary
Our community is facing a true healthcare emergency caused by the federal government. The crisis will hit us soon and worsen in the next few years. The County's hospital and health clinics will be hit hardest, but other critical County services – such as mental health care and public safety – are also threatened. Everyone's care is at risk, as closure of trauma centers or emergency departments increase wait times and affect patient care regardless of what insurance you have. Even if the temporary sales tax passes, the County will have to make hundreds of millions of dollars in service cuts to address a $1 billion+ shortfall. Without revenue solutions, there is no way to avoid devastating local service cuts. Over the next several years, we need to come together as a community to chart a path forward that ensures the health and well-being of our residents, especially those most in need.

Vice Mayor Allan reported that there is an upcoming meeting for the Silicon Valley Animal Control Authority but had nothing else to report.
Councilmember Shannon reported she attended a meeting with the Santa Clara Valley Water Commission. After a short meeting, they went to the Anderson Dam for a tour. Councilmember Shannon shared that the dam is being completely rebuilt as a seismic retrofit project and is scheduled to be completed in 2033. The Anderson Reservoir is the largest reservoir and has been drained for a few years and will not be filled again until after 2033.
Councilmember Mekechuk reported that the West Valley Stormwater Authority and the West Valley Solid Waste Management Authority Board are scheduled to have a meeting this Thursday in the Monte Sereno Chambers. Silicon Valley Clean Energy Authority has a scheduled meeting next week.
Lastly, Councilmember Mekechuk shared he attended a meeting with the Santa Clara County Recycling and Waste Reduction Commission last week. Councilmember Mekechuk shared he believes there is an opportunity to work across agencies and get household hazardous waste out of residents’ garages, sheds and homes in Monte Sereno through the silos across the government.
Mayor Craig reported he attended the West Valley Mayors & Managers meeting last week. There was discussion about the impact that reductions in federal money coming into Santa Clara County is going to affect the cities. There was also discussion on how they are going to implement a 5/8 cent tax across the county to make up for some of that deficit. There is anticipation of a $1.5 billion impact, but this tax is only going to bring in about $300 million. Lastly, the Mayor shared that he, the Vice Mayor and the City Manager attended a meeting online with Caltrans. It’s still unknown who will give the stamp of approval to help with tree removal off Highway 9.
COUNCILMEMBER COMMENTS
Councilmember Shannon commented that last weekend she and the Mayor attended one of the series hikes with Supervisor Margaret Abe-Koga at San Vicente Park.
Mayor Craig shared that at the hike with Supervisor Margaret Abe-Koga, he learned that the woman who owned the land they were hiking on was María Zacarías Bernal de Berryessa. This woman owned land all the way into the Almaden Quicksilver Hills. The Mayor shared there are still a couple of more hikes and extended the invitation for those interested in attending one.
Vice Mayor Allan commented that his number one project as a councilmember will be getting the appropriate contact at the governor’s office to see about tree removal off Highway 9.
CITY MANAGER’S REPORT
City Manager Perkins shared that at the Annual City Picnic, there was an increase in residents’ attendance from last year. There was a total of 210 residents attending this year and last year it was 188. City Manager Perkins also shared that she received information from Silicon Valley Clean Energy today that the board meeting next week will be considering increasing the grant amount by an additional $38,000 to the already allocated amount of $16,000. If approved by the board next week there will be additional money for additional electrification work at City Hall. Lastly, City Manager Perkins, shared that there will be a representative from the County at the next Council meeting to give a presentation on information about expected budget cuts and the Sales Tax Ballot Measure. Residents will be notified, should they be interested in attending.

– Improve financial reporting and human resources management at the City of Monte Sereno
– Utilize time reporting functionality for all personnel and support human resources requirements
– Address and resolve internal control weaknesses
– Develop, document and implement financial accounting policies and procedures with the new financial reporting system
– Convert and migrate minimal, yet sufficient, data across to the new financial reporting system and human resources information system Financial Systems Ad Hoc Committee – 09/02/25 2 Page 25 of 45 Examples of New/Updated/Revised Policies

Reduce/Eliminate Small Dollar Value Checks to Utilities 4 The average value of all checks issued was
$9,250 and the average autopay amount
would have been $309, which is a relatively
low amount to be issuing physical checks for
In Fiscal 2024, using autopay for utility bills
could have replaced 74 checks, or 15% of the
total checks issued
Current State
• The City pays all utility bills with a physical check • Utility bills are all small dollar value, recurring and rarely, if ever, disputed
• All utilities have mature, autopay capabilities, which allows the utility to reduce their billing costs while making it easier and faster for their customers to pay utility bills • With the current policy, monthly utility bills not paid in July are then included in the August warrant list when
there are two months of billing paid (incurring late
payment charges)

Many of the internal control weaknesses identified will be addressed (strengthened) by changing financial reporting processes, and implementing policies when implementing Tyler
An important priority for strengthening internal controls is to have a robust Fiscal 2026 budget to compare actual results with
– Then, of course, having robust financial reporting will enable budget-to-actual reporting each month
Timing – the goal is to have the monthly Treasurer’s Report available for City Council by the third Tuesday of each month for the prior month
– This is aggressive yet achievable

Issuing June 30, 2025 Audited Financial Statements
The June 30, 2024 Audited Financial Statements were issued on July 31, 2025 – A full 13 months after year end
Maze & Associates have scheduled their field work to start on December 8, 2025 – This is much faster than in prior years and is based on their staff availability and the City’s ability to prepare for the audit
Audited Financial Statements lose value as the time to issuance increases – Having Audited Financial Statements available allows decisions to be made with more confidence
The ultimate goal is for Maze to start their field work on the June 30, 2026 financial statements in September 2026

Meeting: City Council - Sep 16 2025
Agenda Heading: NEW BUSINESS
Staff Contact: Gloisy Gonzalez-Langarica, City Clerk
Department: City Clerk
Title: Adopt a Proclamation Declaring September as Childhood Cancer Awareness Month in the City of Monte Sereno
Recommendation(s):
Staff recommends that the City Council adopt a proclamation declaring September as Childhood Cancer Awareness Month in the City of Monte Sereno.
Background Information:
Jacob's Heart led the grassroots effort that established September as Childhood Cancer Awareness Month in the state of California. Jacob’s Heart supports families with a child aged 21 or younger who have received a cancer diagnosis. Families must live in Santa Cruz, Monterey, San Benito, or Santa Clara County in California. Their mission is to improve the quality of life for children with cancer and support their families in the challenges they face.
Outreach Coordinator Allyssa Gil-Ojeda requested the opportunity to be included in the City Council agenda in September and for the City to recognize September as Childhood Cancer Awareness Month.
Analysis:
On behalf of the Mayor, this proclamation Honoring Jacob's Heart Children's Cancer support services and declaring September 2025 as Childhood Cancer Awareness Month in the City of Monte Sereno is presented for adoption.
Staff therefore recommends adopting the attached proclamation.
California Environmental Quality Act:
Adopting a resolution supporting this policy matter is not a “project” under the California Environmental Quality Act (“CEQA”) in that it is a governmental organizational or administrative activity that will not result in direct or indirect changes in the environment (CEQA Guidelines section 15378(b)(5)).
Fiscal Impact:
There is no direct fiscal impact related to the adoption of this proclamation.
Public Noticing:
The City of Monte Sereno's Council meeting agenda was posted on the bulletin board outside of Monte Sereno's City Hall and the agenda with this staff report can be accessed on the City of Monte Sereno's website located at www.montesereno.org.
