TO Honorable Mayor and City Council Members
FROM Nicholas Cuevas, Recreation and Community Services Director SUBJECT    Adopt a resolution authorizing the City Manager to execute Task Order No. 32 to the Joint Powers Agreement with the City of Fremont for Fiscal Year 2025-26 Senior Case Management
Services
SUMMARY AND RECOMMENDATION
The City contracts with the City of Fremont for senior case management services and requires execution of an annual task order to continue providing services. Staff recommends that the City Council, by resolution, authorize the City Manager to execute Task Order No. 32 to the Joint Powers Agreement with the City of Fremont for case management services in Fiscal Year 2025-26.

BACKGROUND
The City has contracted with the City of Fremont for case management services since July 1, 1997. Case management services provides assessments and resources to Newark seniors. Referrals are received from the City's Police and Recreation & Community Services Departments as well as the Alameda County Fire Department. As first responders, City and County staff encounter older adults that need assistance on many levels. Case management services often result in the elimination of repeated call-outs for crisis situations that require public safety response. This allows safety personnel to focus on other business within the City, thereby not straining public safety resources, while helping seniors remain in their homes as long as possible.

DISCUSSION/ANALYSIS
Case management is a vital component of the services offered through the City of Newark Senior Services program. It assists functionally impaired Newark seniors, striving to remain in their own home rather than being institutionalized. An assessment is made of the senior’s overall well-being, including health (physical and mental), financial, transportation and/or housing needs, personal safety issues, and the ability to care for themselves, either on their own or with assistance. If appropriate, a care plan for services is developed to assist the senior in addressing their needs and to provide ongoing monitoring of the senior and the services they receive. Case management services provides specialized resources for the City’s most vulnerable residents that would otherwise not be available. Based on the number of cases opened during the current fiscal year and reviewing previous years' case loads, staff recommends continuing the annual appropriation of $5,000. This amount provides an average of one and one half hours of service per week, which has been sufficient to meet the needs of Newark clients.

The data below details the distribution of services delivered during the current fiscal year which assisted 32 Newark Seniors:

Case Management: Eight (8) Newark residents, five (5) men and three (3) women, received case management services. Their average age is 69. The services provided included a comprehensive assessment of their needs, the development of a personalized care plan, and ongoing monitoring and follow-up in their homes as required.

Counseling: Nine (9) Newark residents, with an average age of 67, received mental health counseling. Counseling services featured a comprehensive assessment and individual therapy, which were provided in the clients' homes. Additionally, various therapeutic groups were offered. Through the senior peer counseling program, Fremont extended support to eight (8) residents of Newark.

Caregiver Support: 15 Newark residents received caregiver support services. The caregiver program assists families who are caring for an older adult and provides support through both group and individual counseling sessions.

FISCAL IMPACT
Funding for senior case management services in the amount of $5,000 has been included as part of the City’s 2024–2026 Biennial Budget.

FISCAL IMPACT
Funding for Newark’s Paratransit services is supported by sales tax revenue from Measures B and BB, administered by the Alameda County Transportation Commission (ACTC). For Fiscal Year 2025-26, staff recommends funding in the amount of $353,727 from Measure B and $14,000 from Measure BB, for a combined total of $367,727 to support both the Paratransit and Meals on Wheels programs. This recommendation is based on Measure B reserves and ACTC’s projected revenue (see attached). An appropriation for this amount was included in the ACTC Measure BB – Paratransit Fund (Fund 286) as part of the 2024–2026 Biennial Budget.
