1. SUBJECT
Agreement with Sunrise Multispecialist Medical Center for pre-employment physical services.
2. SUMMARY
A two-year agreement to provide pre-employment physical services for prospective employees with Sunrise Multispecialist Medical Center for a total of $60,000, from July 1, 2022, through June 30, 2024.
3. RECOMMENDED ACTION
Approve the two-year agreement with Sunrise Multispecialist Medical Center in the total amount of $60,000 for pre-employment physical services; and authorize the Mayor and City Clerk to execute the agreement on behalf of the City.
4. FISCAL IMPACT
The total expenditure for this agreement is $60,000 and will be funded through General Fund (100).

6. DISCUSSION AND BACKGROUND
Since July 2019, the City’s Human Resources Department has contracted with Sunrise Multispecialist Medical Center (Sunrise) to provide pre-employment physical services. The current agreement expires on June 30, 2022. Sunrise has agreed to continue current pricing from July 1, 2022 through June 30, 2024.
Separately, the Orange City Fire Department has a two-year Provider Services Agreement with Sunrise, in an amount of $60,000, for occupational medical evaluations for Fiscal Year 2021-2022 and Fiscal Year 2022-2023.
The City has been able to reduce costs for pre-employment physical services by utilizing Sunrise Multispecialist Medical Center and has been pleased with the services provided.
