AMENDMENT NO. 1 TO AGREEMENT with Serra Yellow Cab for
Transportation Services for City of Pacifica Senior Transportation Program (CaR)
This AMENDMENT No. 1 (“Amendment”) is entered into as of the ____ day of _________, 2024, by and between the CITY OF PACIFICA (“CITY”) and Serra Yellow Cab, a California Corporation (“CONTRACTOR”), whose address is 195 87th Street #D, Daly City, CA 94015.
RECITALS
A. On July 1, 2020, CITY and CONSULTANT entered into an Agreement for Transportation Services for CITY’s Senior Transportation Program. (“Agreement”).
B. CITY and CONSULTANT now mutually desire to increase rates under the Agreement to cover the periods from January 1, 2024 until June 30, 2024 and from July 1, 2024 to June 30, 2025.
C. CITY and CONSULTANT mutually agree that the total fees for services under the Agreement shall remain unchanged at $85,0000.
D. CITY and CONSULTANT therefore desire to amend EXHIBIT B of the Agreement to reflect the increased rate schedule.
AGREEMENT
1. Recitals. The foregoing Recitals are true and correct and incorporated herein as terms of this Agreement.
2. Amendment. City and CONSULTANT agree to amend the Agreement as set forth as follows:
EXHIBIT B – PAYMENT RATES is hereby replaced in its entirety by the amended EXHIBIT B attached to this Amendment No. 1.
3. Agreement in Effect. All other provisions of the Agreement not specifically amended herein shall remain in full force and effect.

EXHIBIT B
PAYMENT RATES
CONTRACTOR will submit an itemized invoice to the CITY by the 20th of each month.

The CITY will send invoiced payment within 30 days of receipt of the invoice. Payments should be sent to:

Transportation rates will be charged at the following rates: $3.10 starting fee (flag drop), plus $3 per mile, plus 15% service fee. Any rides that are less than five miles will be charged the five mile ($18.10) rate. An additional $0.30 click charge will be incurred for trips slowed by excessive traffic or other uncontrollable circumstances. The senior patrons will pay the first $5.00 and the remaining fee will be paid by the City.
Prices inclusive of assisted ride or ride requiring mobility equipped vehicle (able to accommodate wheel chairs and scooters).

The above-described rates are amended as follows for the time period specified below:
• Effective January 9, 2024 and through June 30, 2024, the standard rate ($18.10 plus 15%) shall increase to $21.10 plus a 15% service fee • Effective July 1, 2024 and through June 30, 2025, the standard rate ($21.10 plus 15%) shall increase to $23.10 plus a 15% service fee.
• Effective January 9, 2024, the rate for under 5 mile rides shall increase from $18.10 plus 15% to $21.10 plus a 15% service fee.
• Effective January 9, 2024, and to June 30, 2025, the rate for wheelchair rides under 5 miles shall increase to $40.00 plus a 15% fee. Wheelchair rides over 5 miles shall increase to $40.00 plus $3.00 per mile plus a 15% service fee. • Effective July 1, 2024 and through June 20, 2025, rides under 5 miles shall increase to $23.10 plus 15%. Rides over 5 miles shall increase to $23.10 plus $3.00 per mile and plus a 15% service fee.
Traffic delays, stays, and waiting time shall remain at the rate of $.50 per minute.
All fees, taxes and services under this Agreement shall not exceed $85,000.
