PETITION REQUESTING ANNEXATION INTO
CITY OF PLACENTIA ASSESSMENT DISTRICT
AND CONSENTING TO ASSESSMENT
The undersigned hereby declare(s) as follows:
1. He is the owner of the properties identified at the end of the petition (the "Properties") or are authorized to submit this document on behalf of the owner of the Properties.
2. No other persons or entities are required to execute this petition for it to be an effective petition with respect to the Properties.
3. He requests that the Properties be annexed to the Community Facilities District No. 2024- 01 (Old Town Maintenance Services) of the City of Placentia ("Community Facilities District No. 2024-01" or "District").
4. Pursuant to Section 22608 of the Streets and Highways Code, he waives any requirement for resolutions, report, notices of hearing, and right of majority protest with respect to the requested annexations.
5. He understand that, upon annexation, the Properties will be subject to annual assessments in connection with the District. These assessments will be collected annually on the County property tax bill and are in addition to the general property tax and any other taxes, fees or assessments to which the Properties are subject. He consents to the assessments.
6. With respect to the imposition of the assessments, he waives his or her rights, if any, to notice, protest, hearing, and assessment ballot proceeding pursuant to Article XIII D, Section 4 of the California Constitution, Sections 53750 et seq. of the Government Code, or any other applicable law. He specifically states that an assessment ballot proceeding were conducted in connection with the assessment against the Properties, he would not cast a ballot in opposition to the assessment.
