ATTACHMENT “A-1”
FY 2026-2027 KEY PERFORMANCE INDICATORS

Volunteers of America of Los Angeles, Inc. - Hope for Home Interim Housing Program

1. PURPOSE AND RELATIONSHIP TO AGREEMENT. This Attachment A -1 supplements Attachment A, Scope of Services, for the period July 1, 2026 through June 30, 2027 (the “Reporting Period”). AGENCY shall continue to perform all services required by the Agreement and Attachment A. This Attachment establishes supplemental performance targets and reporting requirements for the Measure A Local Solut ions Fund (“Measure A LSF”) and Emergency Solutions Grant - Rapid Unsheltered Survivor Housing (“ESG-RUSH”). Nothing in this Attachment limits any obligation imposed by the Agreement or by an applicable funding source.

2. PROGRAM GOAL. The program’s goal is to provide safe, year -round emergency shelter services that connect people experiencing homelessness (“PEH”) to interim housing, permanent housing, and supportive services.

3. MEASUREMENT RULES. For purposes of this Attachment:
(a) “Unduplicated” means that a participant may be counted no more than once under each KPI during the Reporting Period.
(b) “Placed into Interim Housing” means a qualifying project entry or placement recorded in HMIS in accordance with applicable CITY, LAHSA, and funding-source guidance. (c) “Moved to Permanent Housing” means a qualifying permanent -housing destination recorded in HMIS in accordance with applicable CITY, LAHSA, and funding-source guidance. (d) AGENCY shall not attribute the same participant placement, housing outcome, service date, or cost to more than one CITY Pass -Through Funding Agreement unless CITY approves the allocation in writing and the allocation is permitted by each applicable funding source.

4. MEASURE A LSF PERFORMANCE TARGETS
PEH placed into interim housing - 91 unduplicated PEH.

PEH moved from interim housing to permanent housing - 46 unduplicated PEH.

5. ESG-RUSH PERFORMANCE TARGETS
PEH placed into interim housing - 40 unduplicated PEH.

PEH moved from interim housing to permanent housing - 10 unduplicated PEH.

6. REPORTING. AGENCY shall submit with each monthly invoice a cumulative KPI report showing actual performance against each target, the HMIS or other approved data source used, and any material variance from the expected pace. AGENCY shall submit a final reconciled KPI report within 30 days after the end of the Reporting Period.

7. CORRECTIVE ACTION. If performance materially falls below the expected pace, CITY may require AGENCY to submit and implement a corrective action plan. The targets are performance goals for monitoring and contract administration; failure to meet a target does not, standing alone, constitute a breach if AGENCY demonstrates reasonable efforts, accurate reporting, and timely cooperation with corrective action. Repeated or material failure to make reasonable progress may be considered in exercising remedies available under the Agreement.
