To whom this may concern,
On the 12th of June, 2024, at approximately 1:45 on the clock. As I was making my way back to my residence along the city sidewalk at Lilac and Winchester, I was walking to my vehicle, which is parked on the side of the intersection of Lilac and Winchester. My injuries were received as a result of a fall that was brought on by concrete lifting roughly one and a quarter inches. As a consequence of this occurrence, I sustained injuries to my left knee, which required me to get x-rays. Additionally, I needed x-rays for my right foot. I had serious bruises on my right hand, which led to the breaking of my wedding band. This was a consequence of the incident. Following the fall, I also sustained an impact to my jaw, which necessitated the taking of x-rays of the lower part of my chin. I cautiously stood up, went back to my house, and called my primary care physician at the Inland Family Community Health Center. I made an appointment with him for the 13th of June, 2024, at roughly 11:35 in the morning. All of my x-ray examinations have been finished. On the next day, June 14, 2024, my primary care physician requested that my care be started immediately along with the findings of my test. Due to this unexpected fall, I now require a walker in order to complete tasks that I do on a daily basis, and am afraid to go on the public sidewalks of the city because of some mild post-traumatic stress disorder (PTSD) that was brought on by this accident. In the near future, I may require physical treatment. Additionally, the doctor advised that I remain off my feet for about 8 to 12 days. I took the necessary time, but I adhered to the doctor's recommendations.
Sincerely,
Irma Delgada
